After raising eight of my own children and practicing since 2003, I've learned a few things about pediatric medicine. A copy of the appropriate Center for Disease Control and prevention Vaccine Information Statements has been provided. I/We have read, or have had explained, the information about the diseases and the vaccines. I/We agree to the schedule discussed with Dr. Thornburg for administration of these vaccines.
Introduction: E-mail is one of several options available for doctor-patient communication. Email sends written messages through the internet. The main advantage of E-mail is convenience. Each person can read and respond to their E-mail at a time convenient to him or her. The main disadvantages are a potentially slow response time and a potential lack of privacy.
Patient Consent to the Use and Disclosure of Health Information for Treatment, Payment, or Healthcare Operations. A means by which a third party payer can verify that services billed were actually provided, and. A tool for routine healthcare operations such as assessing quality and reviewing the competence of healthcare professionals.
I hereby authorize use or disclosure of protected health information about me as described below. The following specific person/class of person/facility is authorized to use or disclose information about me. 3. Brian Thornburg DO PA takes all necessary steps to ensure and protect our patient's private health information.
All parents and patients should be informed about the risks and benefits of preventive and therapeutic procedures, including vaccination. In the case of vaccination, the American Academy of Pediatrics (AAP) strongly recommends and federal law mandates that this discussion include the provision of the Vaccine Information Statements (VISs).

Dr. Thornburg has been my daughter's pediatrician since birth. He and his staff are amazing and we consider them not only to be our daughter's health care provider but friends. They are helpful, knowledgeable and care about each one of the patients. I cannot say enough good things about them and we are truly thankful to be a part of this practice.

I have had the privilege to know Dr. Thornburg and his staff for almost 13 years. He was there for me when I first became a mom every step of the way. Even when my son got very sick , Dr. Rusty was there to bring in his experience. He likes to help you more naturally, which I love. They are huge advocates to help children in every way possible. You will be beyond satisfied with this pediatrician's office. There are no better out there!

If you like to be disrespected, treated like a garbage and told what you will do AND if you want to be ripped off with $2500 annual fee for "doctors access" than THIS YOUR DOCTOR ! I would like to also give special KUDOS to Denise that works in front office. she has just very special skills how to talk to people - I did not know god is female in this office and I'm just unfortunate servant. I think she was hired right from warehouse where she never talk to people. I have NO IDEA how they can even work with kids one only knows what can happen.................................

The most amazing pediatrician in the world! Dr T treats us like part of the family. He's available 24/7. He checks up on the kids all the time. He can be medical or holistic depending which route you want to go. He listens to you! Not only does he care for my children's health but he gives us advice on how to raise a family. He has helped me with my children's behaviors. I have never waited more than 10 minutes in the office. Well visits take about 45 minutes because he talks and listens and gives us advice. One time my daughter dislocated her elbow on a Sunday night and his partner Dr. Rusty came
