We are a Skilled Nursing Facility providing Short Term Rehabilitation, Extended Care services and Respite Care. Our Medical and Therapy teams specialize in maximizing recovery at any age. Our patients are challenged by many medical concerns such as orthopedic, cardiac, pulmonary, neurology, strokes, general debility, cancer, wounds, to name a few.
Within our building are four 12-bed units offering quiet, comfortable healing environments. Each room is private with a bathroom and roll-in shower. There are also cozy living spaces where a patient can meet with other patients, enjoy visitors or knit by the fireplace. We are allowing scheduled visits with vaccinated patients at Flatirons Health and Rehab.

A brand new 44,000 square foot facility, Flatirons Health and Rehab is designed for an outstanding rehabilitation experience. The property features four 12-bed "rehab houses" within the building that lend themselves to smaller, quieter healing spaces, which is so important to recovery.

My Mother had a stay at Flatirons. The building is a nice new building, and clean. Seems my mother says the Food is not good and she needs good nutritious food and drink, occasionally she got a drink she liked. my mother is ill, and these are strange times with Covid. Although sometimes there were miscommunications. I think most of the staff was okay.

I am very vigilant about my meds which was necessary because they offered me meds that are contra-indicated even though I showed three nurses paperwork telling me NOT to take them. When I shared this with administration I was challenged and asked to prove it. I am truly worried about patients that are not cognizant of their needs and medicines. I was promised my dietary restrictions could be accommodated. That was totally incorrect. Bring your own food. They never did understand my rehab was pulmonary and not physical.

My 18 days at Flatirons Rehab literally turned my life around. After two knee surgeries within a month (one due to unexpected complications), followed by a sudden attack of pseudo-gout affecting all my limbs, all leaving me essentially helpless, I was depressed beyond words and feeling hopeless. Within two days at Flatirons all was moving forward, thanks to the optimism and support of their remarkable staff. Everyone was supportive, caring and readily available. The nursing personnel, OT and PT staff and even the administrative peolple created a seamless environment that encouraged a positive attitude

My mom was just released from Flatirons July 2020. She was there for roughly a month in the heat of COVID19. This facility has been AMAZING. From the Admissions Co-ordinator, Director, Assistant Director, nursing staff to the CNAs, PT, OT.... The visiting Co-ordinator and front desk staff, all are AMAZING. The dietician was amazingly patient and kind with my mom and met with her frequently around menus due to issues with her blood sugars. I cannot thank the staff enough for making my mother's birthday special and all the care they put into her while she was there. The staff made my life so much

My mother was at Flatirons Health and Rehab Center in Louisville, CO for about 3 weeks. She had fluid in her lungs and had a Thoracentesis performed at Boulder Community Health. (Another excellent facility) She left the hospital feeling very weak and in need of some rehabilitation. I live in north Boulder and resisted going outside of Boulder. Flatirons Health and Rehab had excellent reviews, better than facilities in Boulder. Flatirons Health and Rehab is a new facility that opened in 2015. It feels clean and looks well organized. The staff at Flatirons Health and Rehab are pleasant, respectful

Flatirons Health and rehab was my home for 25 days.
The facility is bright, open and spacious with a homey atmosphere. From admissions to housecleaning,
every staff member was cheerful, courteous, respectful, professional .The room was very comfortable with a great bathroom that you could move around in. That's a plus when you have to use a wheel chair. The nurses were terrific Food was good and Justin was awesome in the kitchen. The social coordinator, Megan did a great job handling all the activities always informing us of what was available.
With the help of the PT's my goal was met.

The worst discharge planning Ive experienced. Took 2 weeks to hear from home health they referred us to. Was a cash pay only HH and no orders, history, hospital discharge forwarded to this group. Called the facility 3 times to talk the doctor providing care for my mother. The first time, nurse talked to me and refused to give MD a note until I told her that I was a physician. Still never got a call back from any provider. Found out mother was only getting speech therapy twice weekly. Was told it would be more intensive care as she had stroke affecting speech and language.

A family member of ours recently stayed at this facility for a few weeks after a pretty bad injury to her leg.
Very simple stuff went a long way in my review including;
- The staff took excellent care of our family member
- the food was amazing! they have a solid team of chef's that whip together breakfast/lunch/dinner everyday and do a great job!
-The place was clean/orderly/well taken care of
Overall I'd say if your staying here for a while, your in good hands with these folks.

My mother was a patient at Flatirons recently after she had her hip replaced. She has never been in a facility before and we were both incredibly nervous about placing her anywhere. The staff at Flatirons Health made the process so easy and they took such wonderful care of my mother. Javier was one of her nurses and he cared for her as if she were his own mother. Marielou her therapist knew that sometimes my mother needs a little extra TLC to motivate her to get moving. There were so many others but their names escape me at the moment. Thank you so much for the wonderful care that you provided
