Prairie Grove Health & Rehab is rich in services offered to our residents. We base our offerings on the unique needs of our residents and we're open to feedback at all times. Transportation services by appointment (to medical appointments, church, shopping, parks and more). Social Services Coordinators to help residents and families adjust to new surroundings, deal with concerns, and communicate needs during your stay.
