We are a small, privately-owned physical therapy clinic that focuses on what is most important: the patient. Since our doors first opened in 2000, our staff has been working daily to provide quality patient care. We believe that therapy is all about the patient. Since our first day of business in 2000, we have strived to provide the best care to help our patients achieve their goals. We aim to effectively communicate with both the patient and their physicians in order to foster a healthy outcome and a meaningful relationship.
Are you a new patient or are in need of forms? If so, this is the section for you. Most of the required registration paperwork is available for new patients to print and complete prior to appointment. Select your insurance option on a desktop and print them out if you need to. Be aware that this company will cheat you. My lawyer settle with them in August 2018 and I haven't been to them since November 2017. They never claimed any of my deductible to my insurance company and my insurance charged me full price for a procedure after I had already met my deductible with yumanh. People need to watch their statements between this company and a ambetter of arkansas. They also charged me more than what was agreed upon and tried to charge my insurance twice for the same visit. Now they are sending me a bills again.
