Dr. Jazayeri has lived in the greater Fort Walton Beach area for over 18 years now. He is originally from central Illinois and appreciates the warm weather here greatly.
Are you a Provider for my insurance company? Personal Care Family Dentistry is a provider for the following insurance companies: Delta Dental Premier, Metlife, Aetna and Many Cigna plans. In addition, we do accept all other valid insurance dental plans and only your co-payment is due at the time of your appointment.

Never again. I took my son based on the reviews to get him looked at for a cap that had come off a tooth. They insisted on a cleaning, well okay sure. Worst mistake that I will never make again. My son who's 9 left feeling like he had done something wrong because they he was referred out to someone else. Not once did anyone come out to tell there were any issues going on during the exam or cleaning.
This appointment was in October of 2020, in April of 2021 they call me saying my insurance never paid them for anything. Mmm... no i called our insurance when the doctors office finally made me aware

At this point, I can only review my initial visit of x-rays, cleaning & consultation. The girls out front are so very friendly & make you feel truly welcome! The hygienist was very gentle & efficient while doing my x-rays. She was able to reassure me & make me feel very comfortable & very glad that I chose this office! She cleaned my teeth with absolutely no pain & well, i just really liked her. Dr. Nick was very gentle & thorough when he examined my teeth. He took his time & answered all of my questions & told me my options without making me feel that he was in a big hurry to get to the next patient

My appointment was for a cleaning and general checkup. Only about 10-15 minuets on actual cleaning which I felt was rushed to be in and out quickly. Sure some cleanings take longer than others and was told I would need a second cleaning. I felt the initial cleaning could have been completed in one visit but was not. I do have a follow up appointment to take care of a couple cavities that are starting and hope this next visit is better and thorough. Initial rating with a 2 of 5 stars.

What a great office of people! Everyone is so friendly and welcoming. Every time I come in it’s good morning Mr. John! It’s a personal connection every time you come in and they genuinely care about you. It’s so awesome! Dr. Nick is an excellent dentist! I’m truly blessed to have found his office! They have really worked with me on everything! Can’t say enough good about this place!

This was my fourth visit. I was there for my six month cleaning. The first thing I appreciate is they consistently stay to the appointment schedule. I have never had to wait beyond my appointment time before being called back. Also the staff is very friendly. My hygienist made great conversation during my visit and explained what she was doing as she went along. I don’t think you’re supposed to enjoy going to the dentist but this time I did. Great job!
