Services
In December of 2006, the Combined Health District of Montgomery County (since renamed Public Health-Dayton & Montgomery County) began collaborative efforts with Kettering Health Network and Premier Health Partners to establish a Federally Qualified Health Center (FQHC) in Dayton, Ohio. At that time, Dayton was Ohio's only metropolitan area whose general population lacked such a facility.
CHCGD's providers create long-lasting relationships with their patients. It is our priority to provide patients with the necessary knowledge about their health and assist them in making the right healthcare choices. Here at CHCGD, we employ a wide range of knowledgeable staff members who can assist our patients in a variety of ways.

The Patient-Centered Medical Home (PCMH) is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams. Research shows that PCMHs improve quality, the patient experience and staff satisfaction, while reducing health care costs.

Community Health Centers of Greater Dayton is recognized as a Health Center Quality Leader for achieving overall clinical performance among all health centers. This award means that Community Health Centers of Greater Dayton is among the top 21-30% of 1,318 health centers in the U.S. based on clinical quality measures in 2019.

They make you wait extremely long for appointments. Don’t arrive early because you will only add to the few hours it takes for them to check you in and call you to the back.
Front desk, Very unprofessional and lost. Doesn’t appear that the front desk knows what they are doing or have any knowledge about the office at all.
Will be changing my primary care office and I don’t recommend this facility to anyone!
