When you come in for your appointment be sure to bring your photo ID, insurance card, prescription, and any other documentation your doctor or hospital may have provided. You can save time at your first visit by visiting the Patient Portal to complete the new patient paperwork before you arrive. Be sure to wear comfortable, loose-fitting clothing and supportive or athletic footwear that will allow you move freely and safely during therapy.
