Services
Release of Information. To whom the information is to be sent - must be complete with name, address, and phone number. A witness signature must be provided by an individual who has verified the identity of the person requesting information. Requests for deceased individuals must include a copy of the death certificate as well as evidence of executorship of the state.

This review is in regards to the blended case management unit on the 6th floor of the building.
My initial interview with the VP was the first red flag that I should have paid attention to. Her demeanor was very unpleasant and her questions made me extremely uncomfortable.
The VP is a bully, dictator, and discriminative against African Americans. She enjoys yelling, screaming, and making anyone feel small when she is not pleased about anything . You will be told right away by the VP if you do not fit into JFK’s “work culture”.
You will be micromanaged, especially if you are an African

I’ve waited a long time to post this review but I think I’m finally ready to talk about my experience. I saw Alex a talk therapist here when I was at one of the lowest points in my life, Alex made me open up to her and made me trust her. I told her things I had never told anyone ever in my entire life. Alex gave me hope that i would get better. Out of the blue Alex missed our scheduled appointment, I called the office they said we’d reschedule and I waited another week, that week comes and she misses that appointment. This happens 3 appointments in a row and I never hear from anyone in this

Came in a few minutes early for a scheduled appointment and I was asked could I wait another half hour. Had a therapist appointment that needed to be rescheduled and I never received a call from the therapist after calling for 3 weeks and leaving messages! Either the program as a whole needs more help taking care of the patients or they need an upgrade on the equipment being used because it seems very unorganized and it’s very hard to speak to someone pleasant about your personal mental health.

Excellent market offering the best selection of bulk items I have found so far. What a surprise to see they even have bulk protein powder. Their selection of organic produce is quite reasonably priced & very fresh. I only gave 4 stars because there are no customer loyalty programs in effect at this time that I am aware of. I recommend this store & will continue to patronise them.

I regret entering this place in the first place. Being done wrong in the past by others, I was careful and clear by stating my concerns initially when I had to through the steps of seeing different people. One thing I won't tolerate is being misdiagnosed!
I had to go through a social worker and the psychiatrist to reach a therapist. However, they had the nerve to write mental illnesses that has absolutely nothing to do with me.
