The CHC opened in 1994 in Asbury Park to improve the health and well-being of individuals by providing high quality, cost-effective, comprehensive health care services. In 2003, the VNACJ CHC became designated as a Federally Qualified Health Center (FQHC), independently governed 501 (c)3 guided by a dedicated Board of Trustees of which more than 50% are patients of the centers.
In 2001, services were expanded to include HIV primary care and early intervention services. Later, in 2005, prenatal, nutrition counseling and behavioral health were all expanded to the CHC's scope of services. Lastly, a much needed dental suite was added to the CHC of Asbury Park in 2011.

Services
VNACJ CHC team includes fully licensed physicians and certified nurse practitioners that specialize in general health care services for the entire family including disease prevention, immunizations, lab testing and primary care. Our physician and nurse practitioners (NP) provide regular check-ups, gynecological exams, chronic disease management and wellness counseling for health issues such as weight, smoking, and cancer prevention.
The VNACJ Community Health Center's Medication Assisted Treatment (MAT) program provides a whole patient approach to the treatment of substance use disorder. Our licensed and compassionate professional break through the barriers present in the healthcare system to ensure that our patients have primary medical care, substance abuse treatment and mental health services available to them leading to their recovery and a happy, fulfilling future.
The Freehold Family Health Center is a unique partnership between the Visiting Nurse Association Central Jersey Community Health Center and the Rutgers Family Medicine Residency at CentraState. The Rutgers physicians care for you both at the health center and when you need admission to CentraState hospital, so that you have the same team of physicians caring for you with no gaps in care.
The VNACJ CHC is a non-profit 501(c)3 organization that provides comprehensive primary, dental and preventive health care for infants, children and adults of Monmouth County for more than 20 years. Thank you to the community members that have assisted our staff in providing quality health care by graciously donating.
VNACJ CHC is part of the Children and Family Health Institute which offers a number of supportive health care services to children and adults alike. These services range from early intervention services for children with special needs to HIV testing and counseling to home visits for pregnant patients.

I burned my foot on Mother’s Day . Monday I called VNA to see me. they said they don’t treat burns so I went to Med Express . Thursday I went to the hospital to have it looked at and they said to follow up with my doctor. That is VNA thou they accept walk in . I can’t get an appointment when I call at 8:00 in the morning to see the doctor to look at my foot

I just wanted to say how nice all the staff are at this place and how fast the service is! At my old doctor the receptionists and assistants were all on the rude side and i had to wait a long time both in the waiting room and the exam room. Not here! I was there no more than an hour start to finish!
