Eden Pathways operates a Comprehensive Home Health Care Agency in the Twin Cities and Metro areas. We provide primarily community-based services that utilize the skills and services of a Registered Nurse (RN), Licensed Practical Nurse (LPN), Personal Care Assistants (PCA), Home Health Aides (HHA), Homemakers, and Companions.
We also provide Private Duty Nursing services. We work together as a team in an effort to restore function, improve mobility, and to enhance our clients overall physical, emotional, mental and cognitive health.

Services
We all work as a team to ensure that you have all the support you need in order to recover from an acute illness, surgical procedure, or as you deal with a chronic condition like diabetes, heart failure or COPD. We undergo a rigorous recruiting effort and offer our staff a competitive complete benefits package in order to ensure we recruit and retain individuals with exceptional talent and skill and also who are passionate about the work.
From their assessment they determine what additional services may be needed (for example physical therapy, or the need to apply for medical rides). Our aim is to ensure than an optimal comprehensive care is delivered. Our LPN's are the team members that implement the care plan and provide day to day/hands on nursing care to our patients in their homes.
PRIMARY RESPONSIBILITY: The Licensed Practical Nurse is responsible providing clinical nursing services to home care clients. Performs focused nursing assessment of clients to determine appropriate interventions to maintain or improve the client's health status. Provides interventions for clients with episodes of potentially life-threatening instability.
PRIMARY RESPONSIBILITY: The Personal Care Assistant is responsible for providing direct care to home care clients. Perform additional delegated tasks, including medication administration, upon completion of instruction and return demonstration. Maintains absolute confidentiality of all information pertaining to clients, families, and employees.
Eden Pathways has launched its "Pathways to Excellence" Scholarship for 2021. This scholarship offers funding to eligible caregivers who are interested in advanced education in various topics/disciplines, and also who are interested in pursuing CNA certification, LPN, RN career paths.

They show flexibility and a real regard and concern for those they provide services to. My most notable example was when I was unable to return home due to a blizzard and could not relieve their caregivers who were caring for my wife. The office team of Eden Pathways quickly made alternative arrangements to cover the unexpected extra hours.

Heidi continues to be an extremely effective communicator. She has a very open, gentle, and encouraging manner that causes my wife to want to do things that are good for her and that she has resisted doing in the past.
She’s extremely generous with her time. She expresses a desire to help me and Linda in ways that I would not have considered a part of her job.
The proof of the pudding is in something I haven’t seen before in my wife Linda...... a real enthusiasm in her expectation of Heidi‘s visits.

Eden Pathways is a great company which has changed my life in a very positive way. Being one of the care givers in this amazing home care company, I’ve come to learn so many things including how to care for the elderly. The clients are wonderful, so are the employees. I don’t consider this job just a way earning income, but also an opportunity to be part of an amazing family.
