It pains me to write this review because they are generally nice people but i really sense theyre trying to sell me procedures that aren't really necessary.
At the end of the day they get paid on us getting procedures, dentists are the equivalent of car mechanics in that you dont really know much about what is going on, but often times they will tell you you need to top up your windshield wiper fluid when you really just needed an oil change and refilling your 90% full windshield wiper fluid isnt needed. i really feel like this was happening here. Here is the extent of my interactions

Having sustained significant trauma and damage on the dental battlefield over the years, I am truly grateful to have recently found the team at Forma Dental. Marina(front desk), Amanda(hygienist), UberSpaceyXRayScanner Girl(who's name I didn't get) and Dr. Alves all helped to put me at ease on my first appointment. They were all kind, gentle, reassuring, and professional. The office setup is clean, ultra-modern, and full of high tech, state-of-the art equipment. They even provide you with their own masks upon entering the office. So cool. If you suffer from severe dental anxiety then look no further

Dr. Alves and her team are wonderful! Marina was quick with scheduling my appointment. Yenny was very patient taking my X-Rays and making sure I was comfortable. Of course Dr. Alves was very gentle with her procedures and answered all the questions I had. Sisi helped me with my insurance and all my financial concerns. I officially feel like part of the Forma family.

They cancelled my appointment 30 minutes before, without even say "sorry". The lady over the phone has an attitude problem, she confronted my right away because I asked the reason of the cancellation, a complete lack of respect, and I thank you this happen before, now I know what are the true values of this place.

This is my first time in a dentist in a really long time and I have to say that I had a wonderful experience!!
Raquel was my hygienist and I loved her, she was very gentle. The office is beyond beautiful, I felt like I was on a Spa. From the moment I set foot on the office I felt so welcomed that I forgot I was on a dentist.

Horrible experience!!.. My Friend and i had an appointment at 10am, I wanted to get a X-ray and a cleaning, they didn't let us inside until 10:30am saying they didn't have any chairs, after that they told me I have to go back outside and wait in the car for another 10 mins, then they called me saying I can now come inside, after waiting for a total of 40mins!... I went inside they put me to sit, then I had to wait another 30 mins, after someone came and did my X-rays, they left me again for another 30 mins! At that point I got frustrated!.. There is an old saying " Better Lucky Than Good" and it is spot on how I found this Awesome Dental Practice. I was lucky enough to be contacted by Dr. Alves on LinkedIN. I was in the need of a dentist so I took the opportunitty to become a patient. Honestly it was the best thing that happened to my teeth. Unfortunately I thought my husband and I had found a dentist to stay for a long time but we were wrong. The place is modern/clean and Raquel (hygienist) is really nice and good, however the new office mgr Sissi is not. My husband had a routine cleaning and they never informed him he had to pay, then a few weeks later, surprise! he received a bill for that cleaning.
He went to ask why and new office mgr "Sissi" got somewhat upset (not professional) she had a defensive attitude, and in a serious tone told my husband that the bill has to be paid given that the services were rendered!

After my visit today I couldn't wait to get home and write this review. I've been going to the same dentist for the last 12 years but it was time for a change. Going in i knew that I wasn't going to get good news after my ex-rays especially after having a baby and not following up on my usual cleaning. The doctor and staff made me feel so comfortable. It felt as if I was talking to a group of old friends. I'm looking forward to my next visit.

I loved Forma Dental! By far the best experience I've ever had. From the second you walk in to when you leave, everyone is extremely nice and accommodating. It's also such a beautiful office space that instantly relaxes you.

Do you dread going to the dentist? And do dentist offices make you feel terrified as you walk in? I found the best place for YOU (the terrified patient) and people like myself (oddballs who actually relax during a routine cleaning and may even fall asleep - yes, I know!).
First of all, the place is very welcoming with brand new spa-like lobby and facilities and the staff (from front desk to the doctor) is fantastic. Top technology is being used to make patients feel comfortable with all procedures being done to them.
