ParkTree Community Health Center is a Federally Qualified Health Center providing comprehensive, primary healthcare to adults and children living in the communities surrounding Pomona and Ontario, California. Receive excellent health care from the safety and comfort of your home with a telehealth or telephone visit.

Not all medical concerns can be cared for with a telehealth visit. Ask the health center if this option might be right for you. You can also receive counseling services via telehealth. At ParkTree Community Health Center, our trusted team of clinicians and support staff are dedicated to providing our patients with the best care.

To be the medical home for the underserved in our community by providing high quality preventive and primary care health services. ParkTree participates in a strategic planning process every three years to best serve the community. That process is underway and will be completed by the summer of 2021.
Dr. David Kadar is a dedicated healthcare professional with more than 15 years of experience ensuring the delivery of high quality services for families in California. Dr. Kadar is a strong advocate for the unserved and underserved populations of our community.

His health care interests include: ensuring equal access to health care services regardless of ability to pay and integration of primary care disciplines to ensure whole person quality care.Dr. Kadar received his Bachelor's Degree in Biology with honors at New York University's College of Arts and Science in 1998.

ParkTree Community Health Center is the medical home for all of your women's health care needs. From routine cancer screening (cervical and breast) to providing screening for sexually transmitted disease, ParkTree provides confidential and respectful services to women of all ages. Screening for sexually transmitted diseases, treatment for yeast infections, chlamydia, warts, and urinary tract infections.
ParkTree Community Health Center is here to provide services to you and your family during times of stress and life changes. Our counselors are here for you and your family. Our licensed clinical social workers (counselors) can assist you with depression, anxiety, and anger management. Or perhaps you need help to quit smoking, reduce drug and alcohol usage or leave a relationship that is causing you harm.

Horrible experience. Time and time again. So many times the staff do not follow through and leave you on hold without answering. Such a hard time. This is my first bad review I’ve ever left in my life because this service was THAT bad.

This is for the doctor's office at the Holt location: I think they deserve better than the reviews they've been getting. They are worth checking out if you are on Medi-Cal and need care. The doctors and nurses are thorough, patient, and really let you take your time in asking them questions. They don't rush you through. That’s worth 4 stars right there. I've been coming here for 3 years. I think the doctors and nurses are professional and nice. I am a satisfied patient. The reason for not giving the full 5 stars is they put you on a long hold when you call. I had to wait 10 minutes on hold just

Dr. And nurses are very friendly and go out of there way to help you except for when it's something simple like a medication refill. Usually there pretty quick and there are no problems. It must have just been a bad day.

I wish I had seen all these reviews before coming here. I feel sorry for the ones that are actually doing their job to their best ability with integrity but to the ones that are not, they are really hurting this medical business. Never have I seen a healthcare workplace so unorganized, rude and aloof; and in many of the different positions.
I want to question the mindset of those people that think they can get away with this terrible customer service and with treating people the way they do. It’s called healthCARE for a reason; they would also not want to go back if the tables were turned.

I was seen at this location by Dr. Desai, OBGYN. I do not recommend. She didn’t listen to what I was trying to explain about my pain & concerns. Turns out now I likely have endometriosis because I saw a doctor who was professional and genuinely listened to me. Dr. Desai did not and I’m not happy with the treatment. If you want a legitimate positive experience with an OB and want to be heard, go somewhere else.

Terrible service. Website says open on Saturdays. They ask you to call Saturday when they know they are closed. Their voicemail is full and you cannot leave voicemail. Stay clear of this provider and ask your insurance company to assign you to another place. When you really need help, they won’t be there for you. How do you expect their service to be good when they can’t even be honest about their hours and the way they deal with callbacks?
Go find another provider and save yourself a world of pain and frustration

Horrible experience!!! They totally got the prescription that my son was to be taking incorrect. If I had not used my common sense he would have ended up back in the hospital with another seizure. It also took 3 calls, 2 days, and over 2 hours on the phone on hold for them to correct it. I was told I could have his blood work drawn if I came in early. We waited over an hour (even though I was told there was only one person in front of him) when we were told that they didn't draw the kind of test that my son needed, went elsewhere, five days later still waiting for results!

It seems to me that there is a lack of communication with their staff who is in charge of the scheduling not only at this location, but also the Park location. They are not knowledgeable about basic immunization timespans (IE when it is appropriate for the patient to come back for their next booster shots)
I understand that your clinic sees numerous patients each day, but if your staff did not make these simple mistakes then the clinic would operate more smoothly and there would be less frustration on both ends. Just like the clinic and their staff, the patients have other priorities as well.
