Located in a quiet neighborhood, we have been serving the Mansfield community since 1963. We are a 100-bed skilled nursing facility and rehabilitative center dedicated to assuring our residents achieve their highest level of function. Owned and operated by Levering Management, we offer both private and semi-private rooms to meet your needs.

Providing outstanding health care and senior living services since 1956, Winchester Terrace is proud to serve individuals within Richland and surrounding counties. Our team is comprised of the finest administrative, medical and nursing, therapy and support staff in the area.

Our registered nurses and licensed practical nurses work closely with our Medical Director, Dana H. Andrews, M.D. and many other physicians and specialists in Richland County and surrounding areas.Our team of Physical, Occupational, Speech, Respiratory, Music and Recreational Therapists provide outstanding rehabilitation services.

Dana H. Andrews, M.D., M.H.S.A. received his medical degree from The Ohio State University in 1991, and his training in Internal Medicine from Indiana University in 1994.He is certified in Internal Medicine with the American Board of Internal Medicine. He has practiced at Winchester Terrace for many years and appointed to Medical Director in 2007.
Winchester Terrace Skilled Nursing and Rehabilitation Center is owned and operated by Levering Management, Inc. Levering Management, a local leader in senior care and senior living facilities in Knox County and surrounding areas since 1956, has received recognition of its services from the local, state and national level.
Our programs consist of licensed physical, occupational, speech and respiratory therapists, specially trained professional staff in cardiac and IV therapy. They work closely with each resident to develop an individual treatment plan. The team's awareness and personal interest in each resident is an integral part of the care plan.

Please do your research before placing your loved one's here... Their were a few employees there that were very caring and loved taking care of my mom. But the nursing staff was not caring towards my mom and it was sad I am glad she is out of there and I will no longer have to deal with a few of the most uncaring nurses I have ever come across...Hope what happened to my mom doesnt happen to any other resident there....

DO NOT send your loved ones to this facility! My sister was in their care for a period of only (7) days and almost lost her life. During the week that my sister was in their care; I had to call the front desk from my home four times to get her some assistance. Had I not made that fourth call to get her some help she would more than likely not be here today. Since that day, she has been hospitalized and fighting for her life. They claim to not be short staffed; yet they consistently walk around ignoring the residents call lights requesting assistance. I say to you again, PLEASE, do not send your

At one time this was a decent nursing home. In the past year it has fallen into neglect. Evidently Levering Mgt. Only cares about making as much money as it can before they close. Don"t put your loved ones here.

Not impressed with this facility. The room furniture is broken, medical equipment doesn’t work and everytime you bring it the attention of an administrator they claim it’s the first they’ve heard about it - despite regular staff mtgs so it seems little to coordination between staff and administrators. One administrator is particular is neither nice nor professional. The staff when in the room are friendly enough but how they interact with each other is unprofessional. I would not recommend this place.

My mother is a resident at Winchester Terrace. I have reported the neglect to the D.O.N. and nothing has happen. I have been a STNA for over 28 years, I have seen the good the bad and the ugly in nursing. But I have never seen it this bad. We turned our mother light on for her to use the bathroom. 2 hours went by, no one came to check on her. The nurse was giving her pills and when mom was tired of taking the pills the nurse asked her " well do you want me to just throw them away?" Where in the world did that become an option? How about putting the pills in pudding, yogurt, or applesauce???? My

The staff, including the nurses, STNAs, therapist, food service, maintenance, administrators and doctor, do an incredible job under very demanding and difficult circumstances.

Do not take your family here if you love them. My grandmother is here now we are working to get her out. The aids have lost all her clothes, they don't change her and leave her wet in the bed, the nurse manager has been told they don't even follow protocol. This place will neglect your family I REPEAT DO NOT TAKE YOUR LOVE ONE HERE!!!!

Worst nursing home ever short staff very lazy had few good employees an you would had to repeat yourself multiple times to the staff for things to get taking care of never in a million years would I ever take any of my family here again
