Our goal is to provide quality home care services to help you maintain independence and live a safe and healthy lifestyle. Through quality assurance checks, ongoing training, employee and client surveys, and data analysis we are able to measure the quality of services being delivered and better serve you and your loved ones.
Self-Direct, Inc. is a NYS Department of Health (DOH) Licensed Home Care Agency and a certified Women in Business Entity, with offices located in Baldwinsville and Utica, NY. We have been providing licensed home care services since 2003, serving nine counties. We also provide Consumer Directed Personal Assistance Services (CDPAS), Personal Emergency Response Systems (PERS), Home Medical Equipment, and a Social Adult Day Program.
From caregivers to physicians, and personal care aides to case managers, communication and care coordination is a vital component in today's healthcare system. Care coordination is identified by the Institute of Medicine as a key strategy that has the potential to improve the effectiveness, safety, and efficiency of the American healthcare system.
The person who stayed on the line while my aunt waited for the first responders was very calm and kept my aunt from becoming worried/agitated. Everyone has been so nice and patient with my mother. At 85 she doesn't hear or see well and when she accidentally hits her button the firemen just make sure she's okay and explain what happened.
Self-Direct is dedicated to delivering client driven and outcome-oriented quality services. Self-Direct has developed and implemented a Compliance Program to assist the agency with achieving this goal. Stay away they don't care about the clients heck they barely care for their workers all the care about it getting their money from the insurances! bill insurance $38+ for aide service then only pay the aid $11 aides dont work or show and can you blame them for that price. this company could care less if a aide ever shows up for you and by the grace of god you get a good aide who does show they will Harrass that aide as much as they can trying to get them to take on other clients or try to make them as miserable as they can calling/emailing/texting them during their shifts. do not hire them

We used Self Direct for my mothers home care for over 3 years. The aides they sent to help us were wonderful. My mother never thought of them as workers but as family. They treated her the same way. There was never a time that we were afraid to leave my mother with one of their employees.
The nursing staff was very responsive and attentive to my mother.
The schedulers knew us so well that we considered them friends.
I hope they read this review, so many times people only complain, never praise these essential workers that went above and beyond for my mother right up until the time she passed

This office is horrible. The owner does nothing but yell and complain about everyone including her family who works there. I would say Pat treats family better but it is far from the truth. She pays them more so that's probably the reason. Caring and compassion is required in this industry and she lacks both.

I worked for self direct for 2 months they was unprofessional unorganized and they HR staff are liars. April is rude as hell and dont care about her staff the way she tries to act like she does. They take advantage of the aides. I was fired after being lied on by staff stating my physical was missing things that wasnt then there'sy sent me a papers in the mail stating i abandoned my job after not being able to get in contact with my doctor to fix the physical that they claimed was messed up. It was just a horrible experience i dont recommend the services to anyone they wouldnt take care of my sick

One of their aides named Kaitlin walked out and left my son who is paralyzed and unable to care for himself completely alone and never returned. When we asked the Director to make changes in their policies to reassure us this would never happen again they blamed it on him and dropped him as a client.

Following my brothers accident (at the age of 38) in which he sustained a severe Traumatic Brain Injury, I wanted to do everything that I could to keep him out of a Nursing Home. Self Direct made that possible. The service coordinators (I worked with two different ones over the years) were very helpful in guiding us through the many difficult processes that we had to deal with on a frequent basis not to mention that they were kind, caring and extremely compassionate. The day program was stimulating and provided my brother an opportunity to socialize with others. When he was not at the Day Program

I have been working for self direct for a year & they are great! They are very understanding with any time off sheets, & if your running late to your shift. They have great staff Kelly Lori & April are some of my favs.

Employees are treated as pawns and are valued by how much they can do instead of the quality of work if you don't cover the many shifts that people call off from your givin an attitude, if the company needs to cut spending aides get the short straw (ex making aides come into the office for a baggie of gloves instead of them delivering a box to the house like they used to, while office staff get paid lunches) I feel bad for the aides that constantly have to use gloves for bed bound clients because a small bag will not last the day.Clients are handled with poor experienced aides from any work background

This place is toxic. The clients are wonderful people who just need help and besides a few good ones this company is horrible to work for and have take care of people you love. So many people call in and they don't find replacements, they hire criminals, the play a huge role is violation to hippa per the employees, they treat you terrible if you don't conform to their idea and they are in just to women who breast feed.
I was fired because I am a breast feeding mother. I needed and was allowed by STATE LAW to pump 2-3 times for 15 minutes. I was pulled from the homes for 2 weeks with no pay. They

First let me state that I am far from impressed with their services. I hired this agency in june 2015, promised a lot of Help. Got nothing! They promised I would have a Aide in July 2015...I didn't get an aide till Oct 2015. My service coordinator , was a complete imbecile did not follow thru on Anyrhing. He also had my paperwork totally screwed up.with another clients info in my records. Self direct, schedulers would cancel my service days with no Aide or replacement! They would place my Aide with another one of their clients! Last but not least this happened quite frequently. Self Direct claims

Hired this agency a year ago, they have assigned aide for you and last minute they call you and Cancel your service aide to put Them on another Case! This happened several times they did this to me. Take note, they did not place someone else for my hours!!. They are NOT A COMPETENT OR EXPERIENCED AGENCY. They give you a Line of B.S that they can Help you with LIFE Line, Wrong! The
Consumer needs to Have Cable hooked up, and be charged $45 month for Life Line?? Along with Cable Bill on top of that $$. Take Note Clients and Family members, Please..Please review what is in your File they keep
