If your insurance carrier requires a co-payment on the services you receive, its due at the time of service. If Dr. Chadalawada participates in your plan, we will submit your claim to the PPO insurance. If there is no response within 90 days, the balance becomes immediately due and payable by the patient.
No, it is an independent practice started by Dr. Chad and is expanding by adding new offices, Nurse Practitioners and Doctors in future. Advanced Nurse Practitioner (ANP) is someone who has extensive experience as a registered Nurse (at least 5 years) who goes to college to finish their degree and passes all the Board Exams set forth by Licensing Board.
Physical: $395.00 -You will be required to pay $100 per person at the time of scheduling to hold your appointment. This is non-refundable. The remaining $295 will be due at the time of your physical. Mandatory Lab Test: You will need to have lab work done. If you have health insurance that will cover this cost for you, you will be given orders to take to the lab of your choice.
