Promise Community Health Center partners with our patients to provide comprehensive, team-based health care. This team approach helps you strive for a better state of health. Promise midwifery team offers care tailored to the needs of your growing family and is designed to support you through the changes you will experience throughout your lifetime.

Oral health is an integral part of the primary health care we provide at Promise Community Health Center. Oral health directly affects so many areas of your life. Promise Community Health Center partners with local Optometrists to bring you local eye and vision care you can trust. Your vision is a big part of your life and we partner with you take care of your eyes.
Services
Promise Community Health Center accepts cash, check, credit card, most forms of insurance, Medicare, and Medicaid. For your convenience, you can also pay your bills online. It may be helpful to have your billing statement in front of you, as it will have necessary information. Payments will be processed within three business days.
If you have an urgent need for medical, dental or behavioral health care during the evenings, weekends or holidays, please call 712-578-8841. The staff of Promise are committed to providing high-quality, respectful, and affordable health care for all patients to improve the health and well-being of the community.
Promise Community Health Center opened in Sioux Center in 2008 and operates as the only Federally Qualified Health Center serving the far northwest corner of Iowa. Promise offers medical care, prenatal care, behavioral healthcare, population health care as well as dental and vision care.

We also offer nurse health coaching, clinical pharmacy and affordable medications, lab services and immunizations, interpretation and translation, and various other services.Our outreach staff will assist you with filling out complex paperwork, help arrange for transportation or interpret for you. We help you overcome whatever barriers you may face to health care.
Promise Community Health Center Family Practice Medical Team includes five providers, Dr. Del Lassen, M.D., Medical Director, and advanced registered Nurse Practitioners, Tana Kass, Emily Leinen, and Roleen Walgenbach. The team also includes registered nurses and medical assistants, also interpreters for spanish-speaking patients are integrated as part of each provider team.
Promise Community Health Center is a patient centered board-governed organization. Our board is composed of committed and enthusiastic individuals from the community, including business leaders, organization partners and patients of the health center. Our board holds regular meetings, volunteers at our events, advocates for Promise and its patients, plans for Promise's future, and oversees its operations and financial committments.

Sent us across the state for a root canal my son didn't need. Thank goodness THAT dentist had integrity and the skill to put in the filling.

One star is being too generous. This is one of the worst places to take your child. I'm not saying the actual dentist is bad, but front staff and management is sketchy. They have called to cancel my daughter's appointment several times, for no apparent reason other than they were double booked, or there isn't enough staff or time to see my child ? They will give you different excuses depending on who you talk to. Then I show up for the rescheduled appointment, and they say they don't have her on the schedule and refuse to see her. They did not care that I drove a distance or that I took the day

Had insurance when I went and seen them and now they are billing me 4 times for 124.00 now I have to call them and I'm not even near them I was only out in Iowa for a while then came back to Washington

I am very disappointed as this is one of the only dentists around us that accepts Medicaid. This does not leave many choices for other places to go, so they feel like they can treat you like badly and unimportant. If you do not answer your phone the first time they call you to confirm your appointment, (they call when normal people are at work).. they will cancel your appointment AND not have the decency to tell you that it is canceled. There are several ways to communicate such as email, text, voicemail, phone call, call to work place. etc.. but they do not care. I drove over 2 hours away for

Honestly the worst place I’ve ever been. I scheduled and appointment MONTHS ago, I arrive today and they tell me that they double booked because I didn’t answer the appointment call. When I scheduled the appointment they never said anything about this. They said to just show up on the day of or they won’t be able to see me for a full a calendar year. I showed up on time for the appointment BEFORE the other person that they booked over me and STILL wouldn’t see me. They stated that you have to call back and confirm. They were unapologetic, wasted my time, and gas considering I drove OVER

Called to schedule an appointment, and they wouldn't schedule one, they told me to walk in. I did so, and after they took all my information, made me wait for close to 2 hours, then told me that they wouldn't be doing any work on my tooth, before they had even looked at it or did anything.
Unprofessional, ignorant staff, they don't help you at all. They won't even schedule you an appointment.

Very unprofessional. Had called to get a chipped tooth looked and I was to come in at 1:30 for a walk in. Well drive 45 min, took time off work and when I get there they tell me that they have no open spots. When i asked to speak to someone in charge they spoke to them. They wouldn't even let me explain that I cannot eat or drunk anything. Once I told them I was going to contact the CEO they decided they had enough room. Very unprofessional and unorganized.
