When your care team feels that you have gained maximal mobility and are able to return home, the social worker and discharge planner will facilitate discharge. A multi-disciplinary Care Conference is held with members of your family to ensure that everyone is in agreement with the course of action recommended.

My father went into rehab and wound up getting Covid and passed away. He was starving when I visited him and never had a cup for his water. There was a pitcher of water 3 times with no cup. One time I went in the hall and asked for a cup the man said here take this I think it’s clean. No, how about definitely clean. My Mom was in after surgery to rehab and the first room had feces wiped down the wall in the bathroom. They also called me to tell me my mother was being difficult And I need to speak with her. Turns out they had the wrong patient. These are only a couple of the issues. There were

First of all I definitely didn't think my family would have the experience they had by placing our relative in "Lord Chamberlain". Their website states they are focused on "the goal of getting residents back to the community" and focused on "meeting the individual's recovery goals". Well that is a lie. My 80+ relative became an amputee unexpectedly and went there for recovery. Prior to this life change, they were driving and doing things on their own. You would believe that when a facility states that they are focused on the recovery goals of the individual, that they indeed would be focused on

Today is Christmas day, and my husband and sister in law waited over one hour for someone to help their mom use the bathroom. She ended up dirty in a diaper, and when my husband left, still no one came to help or clean her. She is in her late 90s, and it shouldn't be too much to ask that she be treated with more respect and care. I wish we could take her home, but we don't have the supposed skill that the staff should have. VERY DISAPPOINTED IN THE WAY SHE IS BEING TREATED.

My mom only arrived a few hours ago, but I can tell already that Wildy, my moms CNA, is WONDERFUL! She’s kind, compassionate, professional, and there before we needed her! She has a sense of humor which in some cases, like my moms, is certainly needed! Hoping that the rest of the staff is as wonderful.

I’m so upset how they are treating my grandmother so poorly . I will be reporting this on. Monday because I’m fed up !!! Thy don’t come when she calls for help to use the bathroom one cna pushed her and hit her in the head the a metal bar while they were weighing her . They are so nasty like they don’t want to do their job! Amongst other things . All she does is cry to me and my grandmother don’t cry ok she’s tough but she can’t take it anymore!!

Today I brought home my 91 year old dad after a 15 day stay at Lord Chamberlain. First, I have to recognize the one saving Grace and that is Pam the Respiratory Therapist. Pam is warm and compassionate and extremely responsive to questions and concerns. She spoke to my dad with such understanding and respect it was heartwarming to see. Most of the specialists (PT, OT ) we’re also responsive and helpful. Now the reason for my one star review. The nurses are horrendous. They are extremely rude and in my father’s words they “treat the patients like they are the enemy”. My father fell twice

My beloved mother has been in LC for about 2 weeks. Many of the nurses, PCAs, and staff are absolutely wonderful. I am disappointed in the lack of clear my mother, my sister, and I have had from the staff. My mother's surgeon has not once been in to see her or evaluate her condition. She wants to be home. One person tells us one thing, another person tells us something different. When Mom would push the call button for bathroom assistance, she would often be told "be right there" and the staff member would never show up. For my sister’s and I, having to admit our dad into Lord Chamberlain was a very emotional decision. We all knew dad couldn’t continue to live alone, as even with an aid every day for 5 hours it just wasn’t enough to keep him safe. He would wait until his aid left to do things on his own and that’s when he would fall, or something would happen. What made this even harder was this all took place during the COVID pandemic and we were unable to visit. Due to COVID, things are different from when my mom was here. We don’t have the face-to-face family meetings, which was always helpful. I know
