At this time, for the safety of the clients, staff and families of Psychiatric Empowerments Services, PLLC, all existing and new clients will be asked to participate in their care by way of Tele-Health services which will be available on a number of different platforms. Services
My experience as a psychiatric nurse has been extensive over the last two decades, and primarily has involved working on inpatient psychiatric units in acute care hospital settings. Since 2016, as I continued to broaden my interest in psychiatry, I became a board certified PMHNP (Psychiatric Mental Health Nurse Practitioner) licensed in the states of Massachusetts and New Hampshire.
I acknowledge that I have had all my questions about treatment answered fully and to my satisfaction. I seek and consent to take part in treatment with mental health provider Jodianne Ingersoll, PMHNP of Psychiatric Empowerment Services, PLLC. I understand that developing a treatment plan with this mental health provider and regularly reviewing our work toward meeting the treatment goals are in my best interest.
The following are the office policies for Psychiatric Empowerment Services, heretofore referred to as the Practice for the remainder of this document. Please read this document carefully. Your signature on this document indicates your understanding of each policy listed below. It is the PATIENT'S RESPONSIBILITY to know (if applicable) their health insurance plan including co-pay amounts, deductible amounts, and visit limitations when applicable.
Currently, Psychiatric Empowerment Service, PLLC's HIPPA notification is available through our website. We ask that you provide an attestation and acknowledgement of receipt and understanding of HIPPA privacy regulations. Please read and understand the HIPPA - Notice of Privacy Practices before acknowledging receipt and understanding.
In adherence to our practice policies, we ask each client to keep a current credit card authorization form on file in the evert that you cannot or do not pay fees that are outstanding or remain as part of your visit and/or late/cancellation fee.
