John DeBisceglie is an Advanced Practice Nurse Practitioner (APRN) specialized as a Psychiatric Mental Health Nurse Practitioner (PMHNP). I began practice in southern Maine (Kittery, Biddeford, and Portland) from 2011 to 2014. I then began practice in Connecticut in 2014 to present day. I have treated many people over these years in outpatient settings and saw people across many phases of the life span.
Below is a list of appointments offered. Please read this information carefully. For initial evaluations, an intake demographic form will be e-mailed to you for completion. These appointments are 60 - 90 minutes. The initial evaluation is our first appointment to initiate treatment. Please bring any pertinent previous treatment records, a list of previous medication(s) tried, and complete intake forms listed here.
