Project Transition
We offer long-term behavioral health support and services for individuals who have a history of serious mental illness (including dual diagnosis) who desire a future that is meaningful, living in the community on terms they define. At Project Transition you are not a patient, you are a member of a community.

You are not a statistic defined by a chronic mental illness; you are an individual with an persistent condition who can learn to function, cope, and recover with the right setting and tools. When provided with a member-centered treatment for mental illness and recovery plan, you can achieve your life worth living.
Services
Since 1982, Project Transition has had experience working exclusively with society's most vulnerable Members - Adults (including young adults approaching their 18th birthday) who struggle with Serious Mental Illness, co-occurring Substance Use Disorder and/or a Dual Diagnosis of Intellectual/Developmental disABILITIES (I/DD), and Behavioral Health challenges.
Project Transition regularly works with Medicaid consumers. We are in network with over 40 counties in Pennsylvania and BlueCross BlueShield of Tennessee. Members of Project Transition have diverse backgrounds in all facets of their lives. We provide psychiatric treatment for adults (between 18 and 65) who are chronically afflicted by severe and persistent mental illness. Members may also have a substance use disorder, suffer from trauma, legal challenges, intellectual developmental disabilities, and physical comorbidities.
Since 1982, Project Transition has been providing behavioral health recovery supports to thousands of members, there are few questions we've never been asked. Below are some of our most frequently asked questions. However, we always welcome more and are happy to discuss any concerns or curiosities you have.
Project Transition's community-based programs provide daily therapeutic treatment and psychiatric rehabilitation services to support the recovery of our members. Commonly referred to as our "Traditional Program, " members reside in an apartment complex that is coupled with a recovery center on the housing's premises or a location nearby.
