Landmark Memory Care
Our family has created Landmark with Memory Care in mind.

Everything has been designed for the comfort, socialization, encouragement and safety of your loved one - from the layout of our community which includes a central park with walking paths for exercise and seating areas for socialization and soaking up the sun, to the raised flowerbeds that enable our senior adults to continue to participate in a favorite pastime - gardening, to the specially designed shower in each room to promote safety while bathing, to the carefully placed call bells and wander guard systems - you and your family have been our utmost concern.
Services
Private Room and Bathroom, all wheelchair accessible, decorated with your loved one's cherished belongings. Staff educated in ALZ and Dementia care are on site 24/7 and are paired with your loved one. This continuity of care provides an opportunity for relationships to develop and enhances the ability to recognize any changes that may occur.
Music is powerful, it can create a thread of memory - helping us to recall happier times, and taking us to a place in our lives that we have long forgotten - even into the very late stages of the disease. Music is stored as procedural memory which is associated with rhythm and repetitive activities.
On this site, we will keep you up to date with detailed information about Landmark Community events, entertainment, family holiday dinners, birthday celebrations and other important activities and notices. Follow our blog page to get up dates about happenings at Landmark! Join our family member mailing list to receive monthly updates on the happenings at Landmark!
We look for ways to connect with your loved one, knowing that putting a smile on their face will put one on yours also. A memory box outside of each resident's room contains a brief LIFE STORY which provides information that will generate opportunities for conversation. Personal keepsakes placed in the memory box create connections to the past for your loved one and provide identification that this is home.
There is a universal truth we must all accept: one day, we will die. When we die, what happens to our physical remains, our life's work, and everything else we leave behind is left to those who cared for us. In some cases, it is left to strangers to execute the wishes of someone who. When you or a loved one is diagnosed with a disease or injury which affects memory and personality, there can be a feeling of overwhelming information.

This is a follow up review to the one I did about a year after my mother, Earline, moved into Landmark. You can read about my many positive experiences on that review, which continued consistantly throughout her time there. In November 2019, mom passed away peacefully and I wanted to share a bit about that phase of her care at Landmark.
During such a trying time, it was extremely helpful and comforting to me to have the frequent check ins with the staff, particularly with Deborah, about the timing of starting hospice. She helped me move through so many difficult decisions and supported me so compassionately

Nice building and rooms with good food. But, do not use their medical provider Wisdom Heath because they overmedicate and misdiagnose. keep your own medical provider. A lot of turnover in staff which makes for difficulty in continuity of care.

I have been most impressed by the attention and caring attitude I have observed over an extended period by the staff at Landmark Memory Care in Lafayette, CO. I have personal experience with loved ones at four other quality care centers and I can attest that the care my loved one receives here is far and above the norm. This is truly a rare find for those seeking a consistently compassionate and caring home. I believe it stem from its small size and family philosophy.

Unbelievable that I was able to find this place! As a caregiver for a parent and an RN, I can honestly say, there is no better place to place a family member. The Hanson family took great care in deciding to develop this business and providing a caring and nurturing environment for my mother. I never once felt like I was putting her somewhere that was substandard. Assisted Living’s are not mandated by the state to have an RN on premises like a traditional nursing home but these guys not only have one, but two very capable and caring nurses available at all times. I have never had a question or

My mother-in-law has been there for 8 months. She receives personalized attention and loving care from the staff. It is a small family owned & operated memory care facility. We're very fortunate to have it in our community. I'm a Nurse Practitioner who has been taking care of patients for over twenty years. Trust me. I know. You're fortunate if your loved one is lucky enough to wind up here. They do a great job. Thanks Landmark for all of your loving care.

I've consistently had a great experience with having my mother at Landmark, receiving their compassionate care. She was the first resident to move into this beautiful facility just over a year ago. She has done far better here than in any other facility she's ever lived in the past. No more endless trips to the ER because of falls and the multiple things that can go wrong when staff are not as attentive as they are at Landmark. I attribute this quality of care to it being a family run center, by one of the most loving, heart centered families you'd ever hope to meet. I trust

My mother in law, who suffered from Alzheimer's Disease lived at Landmark Memory Care for the last 11 months of her life. The Hanson Family became our family and treated my mother in law as one of their own. She received wonderful care and lived in a loving and nurturing environment. I can't thank The Hanson Family enough for all they did to help my family!

I took the tour, before the grand opening, and was so impressed with Deborah's passion for providing a safe, comfortable environment for our family members. I especially love the red toilet seats! That was something I had never heard of before, but Deborah has done her research and made sure this home is helpful to people who are experiencing memory loss - from the color of the dining room walls (to stimulate appetite) to the color of the bathroom doors... truly amazing.

Landmark was built with only the quality of life of the residents in mind. Deborah and Peter thought of every detail, down to the color of the plates and walls to help their residents feel more comfortable and enjoy their home. Landmark shows as a labor of love. Truly a family owned community. Love Landmark!
