Empowering adults with Neurologic Conditions to experience outdoor adventure in beautiful Central Oregon!. Stroke, Parkinson's Disease, Multiple Sclerosis, Brain Injury, Spinal Cord Injury, Cerebral Palsy, and other conditions. We are Neurologic Physical Therapists who are passionate about getting people into the outdoors with improved physical ability, movement and increased confidence.

We schedule individual sessions (outdoors at local parks) where we can focus on specific things that are limiting your ability to do the activities you enjoy. Our team also offers group programs, providing guidance in a social and fun environment where you can meet others who are experiencing similar challenges.

Carol-Ann founded Destination Rehab as a 501(c)3 in 2016 because she saw the need to help adults with neurologic conditions get back into the community and enjoying nature again.Carol-Ann received her Doctor of Physical Therapy from Duke University before moving to Florida where she worked as a Physical Therapist in a Neuro specialty center (Brooks Rehabilitation).

Join us for a day of exciting wellness, sport and outdoor activities specifically created for people with physical challenges and disabilities. Attendees may have conditions such as: stroke, brain injury, spinal cord injury, multiple sclerosis, cerebral palsy and Parkinson's. Longboard Louie's is serving lunch for all, thanks to our generous lunch sponsor, Bellevue Healthcare!
Traditional physical therapy is incredibly important to the recovery process and we are not trying to replace it, but to provide an additional avenue for accomplishing therapy goals. Most of our activities are based outdoors or in the community, so we are able to utilize natural barriers that are often not available in the clinic environment.
Innovative, out-of-the-box approach to Physical Therapy. We take the clinic into the outdoors, so that you experience recovery in the most dynamic and challenging environment. You get to enjoy the outdoors while improving your movement and health! We offer one-on-one appointments in the Bend-Redmond area parks and we are here to help you on your journey to greater health and movement!

I have had MS for for 25 years. My symptoms and losses seemed inevitable to me. Rachel at Destination Rehab COMPLETELY changed my life, energy and abilities. She was able to use both her skills as a PT and understanding of personality (mine is too goal oriented and driven) to help me stop falling, find better balance in walking and my life and help me with vertigo. Our (my husband and I) experience with Destination Rehab was a wonderful one. We had recently moved back to Oregon after a lengthy absence. Dave was having problems with balance and movement. He had tried a Rehab where he had a gait belt attached and a therapist would walk him around the therapy space. He had exercises to do. We would come home. We tried some Parkinson's dance, etc. But honestly it wasn't a good match. Then we discovered Destination Rehab. What a difference this made in our lives for the next two years. Destination Rehab is a creative, inspiring and empowering physical therapy program that takes people from the clinic and home environment to the outdoors! Using tools like kayaks, hiking trails and even the snow as “equipment” changes the experience and provides people with real life therapies that can be done almost anywhere with anyone!

Destination Rehab is a PT experience like no other. Instead of being stuck inside doing the same work and exercise repetitively, my mom is given a tailor made hour of physical therapy at various parks around town.
