Services
Centinela Skilled Nursing West is a 59 bed SKILLED NURSING FACILITY with an attached 96 bed assisted living facility. We provide quality health care and compassion to every individual who walks through our doors. Upon admission, residents are assisted by a concierge throughout their stay, whose primary focus is to accommodate their needs.
Our THERAPY PROGRAMS help patients reach their full rehab potential and have a rapid recovery. We offer professional rehabilitation services, such as physical therapy, occupational therapy, and speech therapy. With a specialization in post-op orthopedic care, we staff professionally licensed therapists experienced in working with patients recovering from hip and knee surgeries or stroke accidents.
He has been a resident of Centinela Skilled Nursing since then. He has no family in California. His mother is 87, lives in New York, and has been unable to travel to see him. I reside in Ohio and the minute we learned of his condition, I flew out to see him in May. As you might imagine, we were deeply concerned with what happened to him and how he was being cared for.

This place is horrible!!! My father went missing and they had no idea. They noticed around 11am that he was missing did not contact our family until 4pm, they also did not contact police. He is still missing!!! DO not allow your family to go there!!! It is shady all the way!!!! Inside is not the cleanest either, not sure how they are still in business and not shut down. I gave 1 star because I can’t give ZERO

When a close family member broke a hip, we were looking for post orthopedic care to help him become mobile and heal as expeditiously, as possible. The rehab care was excellent, the staff, extremely accommodating and he was able to return home and resume all of his activities even better than before. We were so concerned, because we always had heard, that once someone breaks a hip, there are so many complications that can follow. Centinela was a crucial and integral part of his recovery. The Occupational and Physical therapist were superb .
