Services
We request basic information for our administrative records and to allow us to bill your insurance. Please arrive 15 minutes prior to your scheduled appointment to fill out the appropriate paperwork and bring in a list of your current medications and allergies. If it is necessary for the physician to see old operative records or x-rays, please bring them with you.
While you are ultimately responsible for any charges incurred with your treatment, we are happy to bill your insurance for you. Please be prepared to tell our staff which insurance plan you have so we can verify our participation in the plan. This includes specifying your medical group or if you are in an HMO.
580 - Exit on 150th Ave, turn right and proceed to E. 14th St. Turn right and drive about one mile until you see signs for San Leandro Hospital on the left. After the hospital entrance, the two story brown building on the left is the Medical Arts Building. You can find parking on either side of our building, or at San Leandro Hospital.
