Services
Cash - This includes personal checks and money orders. Care Credit- We work with a flexible finance company to offer you (with approved credit) a way to spread your payments out for up to 12 months interest free. Please ask us for a brochure for more information. In our state of the art facility, all instruments are individually packaged for your protection and all sterilization is monitored with spore testing on a weekly basis.
We have made our patient forms available to you (below) in Adobe Acrobat format. For your safety and convenience, we recommend printing and completing these forms. Please remember to bring your completed forms with you for your first appointment. If you have any problems getting these files to print properly, please call our office and we will gladly send or fax them to you.
