We will provide you with a form that you can submit to your insurance company for reimbursement. Consistency with your appointments will result in faster progress with your treatment.
We all know that life happens, though, and occasionally you might forget your appointment. Our office policy asks that you notify us within 24 hours if you need to cancel or reschedule your appointment. We will charge for the entire appointment after the second missed appointment.

We accept cash, check, Master Card, Visa, American Express and Discover. Payment is expected at the time of the visit. We are not part of any insurance plan, meaning that we are considered an out-of-network provider. Check with your insurance company to see what they contribute towards an out-of-network appointment.

Seriously a Pill Pushing Mill..... Nothing to say but WOW. Last time I visited I was charged for a 30 minute session but I only saw the doctor for less than 20 minutes.
After getting the prescription, I went to have it filled only to be declined for the dosage. When my pharmacy and Insurance contacted their office, Dr. Welty said I was no longer a patient. I tired to call the office directly, but they instantly said the issue was my own, and brought up events in the past as their reason for not claiming why I was no longer a patient. I had seen the doctor in September of 2017, and given 3 prescriptions
