Smoketown Family Wellness Center (SFWC) is an integrated healthcare model to support parents to raise children to be healthy in mind, body, and spirit. The Center provides medical care for children and healthy lifestyle support for the whole family in a community-based setting. Like many cities, there's a 15-year gap in life expectancy based on the neighborhood where you live.

Wellness Classes including parenting, cooking, financial literacy, exercise, and so much more!
Louisville's first African American neighborhood, Smoketown, has a strong history of heritage, pride, and community. Once a thriving community, Smoketown has been impacted by many societal ills and has experienced an attrition of resources. Smoketown Family Wellness Center works to bring needed resources back into the community with the aim to increase overall wellness for youth and their families.
Hello, my name is Ellen Sloan, RHIT, CNA. I am a native Smoketown resident and I love my community. My roots go deep. The passion for my neighborhood extends in every direction possible. I have been a Power Member and Change Maker with Network Center for Community Change, and I am a member of the Smoketown Neighborhood Association and The Kentuckians for the Commonwealth.
