The purpose of this Confidential Estate Questionnaire is to assist your heirsand significant others in the orderly and expedient administration anddisposition of your estate by providing them with essential information to locate all of your assets and not miss anything. The biggest problem we face, and your surviving family members have to deal with, is not legal issues, the first questions we hear are "What did they have?"
Proper and complete estate planning is more than just paper, it is about teamwork. The most important tool in estate planning is the telephone. ALTA Estate Services created the Emergency Hotline Telephone Program over 20 years ago.
This program allows ALTA clients, family members and significant related parties to contact ALTA (and/or ALTA contact such related persons) AT ANY TIME AND DAY during times of medical and life crisis. We offer this program to all of our clients AT NO CHARGE and as a courtesy because we know how important this service is for our clients and family members during times of medical crisis.

HIPAA requires certain legal language as part of the Privacy Rules: Powers of Attorney; drafting Privacy Notices and HIPAA Compliant Authorizations and resolving issues of privacy involved in providing services to individuals. Like all new regulations there is a fair amount of interpretation that is required and there are many unaddressed issues in the HIPAA Privacy Regulations.
The Family Limited Partnership (FLP) is probably the most effective entity available for asset preservation and asset protection, and estate planning. Although you "can't take it with you, " by placing your assets into FLPs you can legally and successfully protect everything you own from attack by creditors.

My husband and I are in our mid 30’s with 2 little ones and felt it very important for our family to take time to map out our estate should anything happen to us. Obviously that is inevitable and we wanted to make things easy for our kids and family when that time comes. Working with Mark and Alyssa has been wonderful. They are extremely kind people who truly love what they do and want to help during what can be an extremely difficult time. My family has used Alta Estate Services to set up two Family Trusts. I fully trust them and am comfortable going forward that my sons will be aided at the time of my passing. My experience with Alta and the whole team has been fantastic. My elderly parents set up a family trust with Mark 8 years ago and due to their declining health wanted to revisit the documents. Numerous updates were needed to several parts of the trust because of changes in the law. Mark and is son Marc both worked efficiently, thoroughly, and compassionately to make sure all my parent's wishes are legally documented and accessible when the time comes. We have been working with Mark Fishbein for approx 9 years. They were very professional when we started an estate with them and even though we neglected to do an annual update, they were still very professional and assisted in completing a new trust which gave my wife and I piece of mind. We were referred to Alta Estate Services by a friend when we needed to update our estate planning. Mark met with us and explained the importance of asset preservation to ensure our designated beneficiaries would receive the maximum possible with minimum legal obstruction. He also assisted with out-of-state legal work that had not been properly filed. We frequently travel and if we had an accident or became ill we feel secure in the knowledge that family members would be made aware of our medical needs and any records or authorizations promptly provided.
