Services
A. For your first appointment, we request that you arrive fifteen minutes early to complete necessary paperwork. If you've already filled out the New Patient Form, which can be found here, we recommend arriving just five minutes prior to your appointment. Please bring up-to-date dental and medical insurance cards, as we will make copies of both to have on file for future reference.
Until your root canal procedure is completely finished and the permanent filling or crown is in place, you should minimize chewing on the tooth under repair. Upon completion of treatment, the tooth may feel sensitive for a few days due to natural tissue inflammation. Most patients can return to their normal activities the next day.

Would not recommend for dentistry or orthodontia. Hygienist at Deforest location chipped my tooth and Covid procedures didn’t seem up to par (same gloves touched mouth then mouse/keyboard/equipment then back in my mouth).
They weren’t clear about costs/procedures needed for orthodontia, even when specifically asked. Many appointments, all a waste of time.
They insisted I get a filling that my regular dentist did not even mention I needed when I returned.

Dr. Donnelly, Lindsay my hygienist, and the whole staff at Madison Family Dental Associates are FANTASTIC! I have very sensitive teeth and Dr. Donnelly and Lindsay are VERY accommodating to my needs and take the anxiety out of going to the dentist. They are not pushy for adding additional services and their front office is great about explaining any out of pocket costs. I would recommend them to anyone, and I do!

Horrible experience twice. First teeth cleaning felt like the woman was using a jackhammer on me. Had to specifically request someone else. Multiple lectures on flossing- thanks for treating me like a 3 year old. Just walked out before getting fillings, was very specific at appointment before not wanting to get x-rays due to cost. The doctor was in the room for that- had a quote for the fillings before hand. With the numbing gel in my mouth was informed I needed x-rays or the doctor would not do the fillings. I don't have the $$ right now to add that to the bill, ma'am. I budgeted for this appointment
