Whether you call it an intake, a mental health evaluation, or a diagnostic assessment, the first appointment with a therapist can make people nervous. We do our best to alleviate some of that anxiety by explaining the paperwork you are completing and signing. We offer optional online intake forms, or you may complete traditional paper forms in-office.
Victoria "Tori" Carroll, MS, LMHC, MHP, CMHS is a Licensed Mental Health Counselor at ACT Services and in the State of Washington. She also holds State designations as a Mental Health Professional, Child Mental Health Specialist and an approved clinical supervisor.
