South Mountain Post Acute is a modern, state-of-the-art care center, conveniently located on the South side of the Phoenix metropolitan area in the heart of South Mountain Village. We are a short drive from many area hospitals including Phoenix Memorial Center, Dignity Health Arizona General Hospital, Maricopa Medical Center, and Banner University Medical Center Phoenix.

After a hospital stay, I was hesitant to go to a rehab facility... But it was highly recommended to go. So long story short, This was the facility I was at for 2 weeks, and to my surprise it was a better experience than what I thought it would be. Everyone was great and took care of me. My therapy started my 3rd day there. And I'd like to thank these several employees there: Glenn, Nikiya, Sabrina,Tobias and Tina. I could tell they loved their job and I appreciated them so much... But especially Glenn. He was awesome.

Don't send anyone here! The staff is nice and polite to the family members but behind closed doors they degrade the patents and are very condescending to them as well. When he wanted to leave they told him to just go knowing he couldn't walk which just made him angry. They left him with no buttons for the bed, tv, phone, or even the nurse button. The staff was not sure what they did and didnt have, they helped make sure he was safe in our car when I went and got him but he said they where speaking poorly of us and him in front of him.

Do not send your loved ones here!
My Dad was sent here about a month ago and it was a terrible experience. He died after contracting COVID here!
On two occasions they gave him insulin without checking his blood sugar which subsequently he struggled to wake up from prompting paramedics to come and raise his sugar level. On a few occasions staff was condescending and undignified towards him. This is the worst place for rehab!

What a nice place! The staff were really friendly and hospitable. Tyler is great at his job in helping with transitioning. I liked the layout of the facility as it was really easy to navigate. The staff was very nice and helpful!

My mother was here recently, and from the time she got there, we wanted to move her out to a different facility. We had our own facility picked out but the hospital that she was at decided to stick her at South Mountain. From the start i was told by numerous people in the medical field ( phoenix area ) that South Mountain was NOT the place to be, and that proved to be correct. My mother has very good insurance that would place here in many great facilities. Now getting back to South Mountain, 1st complaint: My mother sat waiting for a ride for 4 hours to come back from dialysis, it was strange

My mom died here 2 weeks ago.After her stroke and a good referral from a "nurse" we transferred my mom here. Had trouble communicating with my mom and getting a call back from the staff. When I would call to get an update they would get upset for asking questions and would tell me she was doing well. Up to the date she died i was told she was good. I believed them because they are the "professionals" i should of followed my gut but is too late now. she died all alone. After talking to her on Tuesday i realized she didn't sound well. I called her nurse.. was told she was well.. On Thursday i requested

I'm at the place now. They served the food forgot to serve mine. An hour later when I said something about it they all acted like I was starting trouble for nothing. They even tried to lie and say that it didn't happen. Not one took responsibility for it. Just acted like nothing happened. Just to be clear they served my food a hour late.I'm at the place now.
