It is well known that Hawaii has one of the highest rates of homelessness per capita in the nation. But besides lacking stable housing, homeless individuals have woefully inadequate access to health care. Often their only source of treatment is a hospital emergency room, But episodic ER care does not provide continuity or access to preventive services while wasting resources that are needed for true emergencies.

H4 seeks to expand access and improve the quality of care for the chronic homeless, while reducing the cost of their care by eliminating unnecessary use of expensive hospital services. The City and County of Honolulu envisioned a model of comprehensive services for the homeless in a single location.
Services
We welcome individuals or organizations with compassion, empathy, and desire to give back to the community and support H4's mission, giving hope to those less fortunate. H4 is looking for volunteers or employees with clinical training and experience, including family practice/internal medicine physicians, psychiatrists, APRNs, registered nurses, physician assistants, licensed clinical social workers, psychologists, and medical assistants.
Referrals to H4's medical respite service will most frequently come from hospitals, but referrals from other medical providers and organizations serving the homeless population of urban Honolulu will also be considered.

H4's clinical team will evaluate the appropriateness of each prospective patient's admission to the unit based on the information provided by the referring provider or agency, and an in-person examination and interview may be required before the final determination is made.4.
