Phoenix Mountain Post-Acute is a modern, state-of-the-art skilled nursing facility, conveniently located in northeast Phoenix, east of AZ-51 and south of E Thunderbird Road. We are just north of the Paradise Valley Mall on N Tatum Blvd behind the Brookdale Assisted Living. We feature 130 beds and beautiful spacious rooms in either private or semi-private settings.
Phoenix Mountain Post-Acute is now offering physical and occupational therapy for Parkinson's and other neurological condition services to current and future clients. We are now certified in LSVT, expanding our range of services to give patients the treatment to accelerate their progress.

The staffs here at Phoenix Mountain Post Acute are amazing. They are all very caring, and extremely attentive to the residents. You are greeted with a warm smile and hello from the moment you check in. The building is beautiful and very clean. I have only been here a little more than 3 weeks. I absolutely love it here. I would definitely recommend Phoenix Mountain Post Acute to friends and family, as I know they will be in greats hands.

TERRIBLE!!! There are no safety rails on the beds so they put mats on the floors when people fall out. Doctor's orders regarding diet and fluids are NOT followed. Ants in the room. Vents from heater/AC unit are clogged with dust. Patients have to beg for water. Some nurses are nice but others are hateful and mean. They lose your property and then claim you never had it to begin with. IF YOU CARE ABOUT SOMEONE'S LIFE DO NOT TAKE THEM HERE. If it was possible to post less than one star I would.

My father in law lived out the end of his life here and all I gotta say is everyone is great I wish I got names but if the nurses read this there was a girl who helped Joseph she was able to answer all questions without hesitation, she did everything we asked and never made us feel like we asked for too much. Another one was a guy with a shaved head he was so gentle. The care was just outstanding I'm glad we chose you guys to care for him and especially the last 2 days giving him his own room so we could make sure he knew he was loved by his whole family, his wife and daughter spent the night with

Angela (Social worker ) at this facility, has Absolutely NO tact , when it comes to speaking with Families who have their loved ones in this so called facility...
This woman called me CLAIMING , Medicare had stopped much needed Therapy for my Husband who was sent to this place for rehab after being hospitalized for serious illness ...
I received Correspondence that states the FACILITY stopped therapy!
Ms Angela was rude and hostile with me after telling me I could appeal Medicare's decision to stop therapy... due to " No Progress"...

One of the worst places you could send your loved one. My father came here after brain surgery and ended up falling out of bed. This place *REFUSED* to have him taken to a hospital to have him checked out. After BRAIN SURGERY. After hours of complaining to administrators, they finally sent him, wearing NOTHING but a shirt and diaper. No socks or shoes on a cold night in February. They lost his clothes so regularly that we had to pick them up and wash them ourselves to ensure they weren't lost or given to other patients.

I am only giving it one star because there's no choice for and negative star. My dad was in that place for a week and they are very unprofessional. They took his clothes and he did not receive them back until 3 days later. He said he could never tell what he was eating and the food was always cold. I would never recommend this place to anybody!

I have placed my brother here two different times and made my selection the first time because of it's location, and this last time because of their efficiency in making a plan for Michael's best care and communicating his needs and progression of rehab.

I took my mother out of the facility as soon as she told me that person called 911 in the middle of the night do to lack of attention, she was in pain and there was nobody helping her, my mother had issues with some of the services, she had to wait to go to the restroom and by the time they got to her she it was too late :(

Typically I do not take the time to write reviews but feel compelled to do so regarding the audacity of this facility. My father suffered from two massive strokes November 2015; due to tremendous problems with the hospitals it was time for him to transfer to a skilled facility. Several places including this one denied my father due to his cussing at nurses and refusals to do therapy. I attempted to call and speak to admissions which I believe his name was "Greg" and explain the situation and see if he could see it from the family's perspective. Me and several family members were

For the most part, the staff is great. They have tough jobs and I applaud them for doing it with patience and grace. On the other hand, there's a nurse here, Princess is her name, who is rude and abrasive to family, visitors, and patients. I would have rated this place a "5" until I had to deal with her. If you don't have to deal with her, this is a fine establishment.
