These positions' primary duty is to perform the appropriate nursing care level to nursing home residents for a skilled nursing facility that includes a Sub-Acute unit. I had a friend that got transferred from the kindred Hospital of San Diego to the Jacob's healthcare center before returning to our home they pushed her as fast as I could to get her out of the facility before she could even get up and down from the bathroom or use her arms to get in and out of the bed all they cared about was her ability to walk and she could walk with a walker but she cannot get up or get down due to her being pushed out of the hospital 24 hours later at her home she is bedridden she is in so much pain she cannot get up and down at all and because of her size we are unable

I am very happy that we decided on having my partner move to Jacobs Hlth Care Cntr. The staff is very professional. They are aware of individual needs. They work closely with you to ensure that they meet the needs of individuals that are within their care. No place is perfect. But I've found at Jacobs, communication is imperative in having to address the different issues that may come up, especially when you have so many different types of clientele/patients that they receive. I've had no problem with the staff. It's all about finding solutions. Thank you very much.

I worked at this facility as a DSD but have since resigned. I find the Management team extremely caring and compassionate. Kudos to Derek the Administrator. He made me feel right at home the first day. He ensured my Orientation was more than I could ever have expected. I felt listened to, respected and treasured. Not many facilities go this extra mile. The Nursing staff rock and truly care about the basics. I would have my family cared for in this facility.

It's interesting how the staff won't respond to negative reviews but be sure to pass on 5 stars to the team. Call it like it is. Jacobs is known to allow patients to suffer. If they aren't getting the money or funding, the care they provide is minimal if that. Unfortunately after a state investigation, it was determined that paperwork was forged by the staff. Additionally, medicine is given that is harmful to patients that is not documented. When a patient proves medicines, with evidence, that are given that were never prescribed, given to them, it shows they are manipulating their

The team at Jacobs has been phenomenal helping my wife recover. PT, OT, speech, nurses, sub acute section and SNF section all have been so proactive to help make my wife better. They are so dedicated, creative and keep patients and carer needs in mind while still being so so proactive.
Would recommend anyone with someone coming out of ICU or a LTACH to go here for their next phase of recovery.

My Grandmother arrived on April 7, 2021, a hurried transfer from Arroyo Vista who had subpar service. From the beginning, with the tour, someone greeted us with a smile, the rules of visitation were clearly communicated. The spaces are clean and well lit, the energy is positive. Most important, my grandmother has been treated like a queen! She is our treasure so it's nice to see the staff recognize her as such as well. All staff in Jacob Healthcare Assisted Living Facility, led by Administrator Amy J. and her Assistant Jacky I., are caring, compassionate, and considerate to all the residents. In full transparency I have a management role in this facility; however, I have no reservation in saying that we have one of the strongest teams in the county. We do our best to put ourselves in the resident’s shoes to develop solutions that serve their needs. We are far from perfect but we hear staff and residents and resolve concerns as fast as possible. Staff are happy and I know that because we speak with them individually and the staff stay year after year. And as we know, happy staff give great care.

I recently had the opportunity to obtain care for my mother by the staff at Jacob Healthcare center. It was a wonderful experience and the staff were very professional, efficient and attentive. They were very receptive to her needs and right from the front desk staff to the administrative personnel and nursing care everyone was top notch !! The premises are also neat, clean and well maintained ! I highly recommend their facility for anyone seeking a skilled care facility.

As I suspected, the abnormally high amount of stars is not a true representation of the business, but the business cherry-picking which reviews to fight or leave with Yelp. After my foot surgery i picked this place it was the worst decition i have ever made, everything that they promise to have the didn't have, if you're considering this place for your own good i urge you not to go to this place after 1 day i had to go to bella vista health center if you really value your health don't go to jacobs if you want to know how they treat you in bella vista health center see my review of them.

Basic care is minimal at best. Want a shower or bath?
You'll be wanting for weeks
Smell. We'll let that go.
No communication.
I could go on, but to what end?
Avoid if you love your significant other.
If you are ambivalent, put them here.
And - for all you folks that need to know -,cost per day was estimated at $425. Yup. Roughly 3K per week. 13K per month.
Motel 6 is less than $100 per day. Decide.

This is a negative update, as the Assistant Administrator insulted me, said lies on me, didn't offer an apology for the Discriminatory actions that the CNA's and other staff members have treated me with. Including mistreatment, neglectful, emotional and mental abuse. Instead she said to not think of the past 8 months, while the staff has current and present actions in the past 7 days I reported to their supervisor. Now, the Assistant Administrators Boss has failed to contact me for this entire week. My body has became healthier in the past 2 months, yet that is not a Positive welcome

Jacob healthcare is not only a beautiful facility, but also one that truly cares and advocates for all of its patients and residents. I have had the pleasure of developing personal relationships with the administrative staff as well as the nursing staff and would have no hesitation sending my own family members here. Being a hospice care representative, I am constantly asked by families of my patients for referrals to SNFs and ALFs. I have never thought twice when recommending Jacob Healthcare to them as a place to receive care.

This is a wonderful place to bring your loved ones. Being in the healthcare industry myself I have had the opportunity to experience the attentiveness of each staff member at Jacob's, the place is clean, friendly, and there's always fun activities to keep the residents engaged and social. I would highly recommend doing a tour to appreciate all it has to offer.

Dr. Green was the only good thing about this place. I had a family member stay here for about a week before we transferred him to a better care facility. The nurses took forever to respond to a call button. They never reported a fall my family member sustained while there, they took too long to test for a UTI, which we discussed our concerns about in a care plan meeting, only to have him rushed to the hospital incoherent due to infection running through his body a day later. Plus, I have video of one of the nurses, who was supposed to be watching him because he was so out of it, nodding off

Amy Jeffers gave us the grand tour of the place and she's as sweet as can be. She is one busy lady and she is always running around to make everybody happy here. I really admire her hard work and dedication. The facility is absolutely beautiful and the employees are super sweet and helpful. Lewis Hawley is enjoying his stay here. Jacob deserves zero stars. Jacob is a horrible place to send your loved one. My mother was neglected and she passed a week after we removed her from this place. Jacob made it extremely difficult for us to move our mom. The icing on the cake was when I went to visit my mom and noticed what looked like cottage cheese coming out of the sides of her mouth. No one at Jacobs noticed she had thrush in her mouth until I pointed it out. She was not given an air mattress, which is something you do for a patient that cannot move on their own, and as a result developed a stage 2 pressure wound. I agree with

I did my clinicals here for CNA a few years ago and it was just horrible. This place is dirty, smelly, and not worth staying at! Don't get me wrong, I had a great learning experience and met lots of great patients staying here but this facility and the people working there to "care" for the patients need to actually work. Call lights would be on and the CNAs there would just walk past them and pretend not to see the light. Also, there is no proper infection control and turning patients to avoid bed sores seemed to be ignored as well. I witnessed

My sister lived at Jacob Healthcare for almost 2 years. I have nothing but respect for all who work there. They all care very much about all of the residents and they all took very good care of my sister.
