From tolerating everyday activities to finishing this year's marathon, we join your journey to get you back to you. Sessions take place in a boutique-style setting where patients can expect an attentive, holistic, one-on-one approach to their care.

FreeForm utilizes a whole body method to not only treat symptoms at the site of injury but to address the root cause of pain. Omer Aguilar has spent his entire career in orthopedics and sports medicine since completing his Physical Therapy degree at Wayne State University in Detroit, MI in 2001.

Friendly people and clean space with such a positive energy! Zach has helped my neck and back pain so much- I was very nervous and skeptical coming in for my first time, and he made me feel comfortable and I felt heard!

Omer changed my life. I would give ten stars if I could. In four months, he almost completely eliminated my extreme neck and shoulder pain that had been with me for 25 years. He was patient and friendly and explained everything. I received dedicated one-on-one time with him during every visit. He is a true expert with a wonderful bedside manner. Omer is an outstanding physical therapist. He helped fix my muscle imbalance issues and really listened to me when I was going through a few rough patches of running related injuries. So happy to have found him !

Happy to have a knowledgeable physical therapist that I can bring my kids to. Omer treated my 2 year old when she popped her elbow out. Dealing with a frantic mother and a crying toddler, Omer was patient and kind and was able to treat my daughter quickly and efficiently without her feeling scared. Since then he has treated my kids for a variety of maladies and has saved us from a trip to the ER a few times. A+ service. I highly recommend.

Great experience. I had two different but somewhat related joint issues. Omar asks great questions to diagnose the root causes. He was able to design an exercise regimen that was effective at relieving both issues while not being overly time consuming. I highly recommend him.

I've seen three different PTs over the last few years for runner's knee and other issues. Omer is the only one who's been able to successfully get to the root of the problem and get me back running, feeling good. FreeForm's facility is great -- easy to find, clean, light and a happy, positive place. Highly recommend!
