Mason Creek Transitional Care is a modern, state-of-the-art care center, conveniently located at 21727 Provincial Boulevard, near Mason Road. Our small town setting west of the Houston metroplex, serves Katy, and surrounding areas. We are only a short driving distance to Memorial Hermann Katy Hospital and Methodist West Houston Hospital.

My dad went there after he broke his hip. He was well taken care of, the food was great and the staff is so very sweet. Rob is the hardest working social worker I’ve ever seen. He arranged for my father to receive palliative care at home due to my sisters wishes. He arranged for an ambulance to carry him home. Nelly is the hardest working nurses assistant. She sings to them and is so very dedicated. I don’t know if my dad wasn’t healed enough for PT but that is why he was here. My sister wanted more PT at home.

If you care about your loved one DO NOT put them in this facility!!!!! The only reason they are even receiving one star is so that I can write this preview.
My father was transferred here on a Thursday evening after he had his leg amputated. We were told we would not be able to see him until Monday afternoon after we took the patient care class. We were told that we could check in with the nurses and that we could reach him on his room phone. We were notified on his first night there that he had fallen out of bed but everything was okay. I was only able to reach the nurse one time during a 36

Exceptional care, devoted staff. Thank you Mason Creek Transitional Care, especially to Kelly, for all of your support and care for my 94 year old mother so she could recover and rehabilitate from COVID. Kelly particularly went above and beyond to give our family reassurance and personal attention in facilitating the transfer and intake of our fragile mother considering we were doing this all remotely during a pandemic crisis. My mother was there for 2 weeks and it was not good at all. She had to be transfer to another facility. When she was transfer they kept all her stuff that she was sent with all her new clothes, shoes and personal hygiene items. My brother called alot of times asking for her things and they always said we look for them and call you back well we still waiting. When she was transferred to the other facility all they sent her with was her back braces only I guess they felt that's all she needed due to her surgery. They are getting 5 stars but for our family it's is a zero due to the fact the commutation
