You will not be kept waiting as we aim to see all patients on time. Your entire treatment will be given by licensed Physical Therapists and Physical Therapy Assistants.

Known for her unique hands-on and holistic approach to physical therapy, Cheryl is most recognized and respected by thousands of men and women in the Intermountain region. Cheryl has been a Physical Therapist for over 15 years after graduating with her Masters Degree from the University of Utah Physical Therapy Program in 1998.

We want to make physical therapy sessions as smooth as possible for our patients. Please arrive at least 20 minutes prior to your initial evaluation to ensure all paperwork and insurance information is obtained. We ask that you please bring a form of ID, your insurance card, a referral from your physician (if applicable), and any copayments you may have.
ALL patient need to verify their own insurance benefits before they come for their initial evaluation, or if there is a change in insurance. We will check your insurance benefits as a courtesy to you, but it is your responsibility to know your own plan! We off a discounted Self Pay rate for those individuals who do not have insurance or we are not in-network providers with their insurance plan.
