City Creek Post Acute is a modern, state-of-the-art skilled nursing facility, located just east of downtown Salt Lake City. We are 1 block south of the Historic Salt Lake Regional Medical Center. We feature 108 beds and beautiful spacious rooms in either private or semi-private settings.

Most awful place to leave your loved one. They will "misplace" important and very expensive meds and discontinue others without consulting family or even a neurologist. My sister ended up with many seizures and back to the hospital neurology unit because of these 2 medication mishaps.
Most aids are nice and helpful but some are combative and others treat their adult patients like babies. Very off putting.
Administration is also very clueless.
Getting a response from medical records is impossible. Been leaving messages for a copy of the vaccination records for weeks now.

Choosing to go to City Creek Post Acute to recover and receive therapy was the best decision I made regarding my injury. The nurses, CNAs, physical/ occupational therapists, really the entire staff, went out of their way to insure all my needs were met and I achieved my therapy goals. A fun environment with a truly caring staff. I recommend it to everyone looking for a positive experience in a care facility ❤️

I feel sad to leave all of you and yet I am so excited see my loving family.. I would recommend city creek to anyone looking for a great Acute care facility! I thought the staff went above and beyond with the following qualities! Here s what i I saw, and what I felt! Kindness, a lot of compassion, Many smiles were given thru out the day, and evening!, if you called for help, the aide responded in good time. One night my legs gave out almost completely, Carol lee was right there and caught me, called for another aide, Nigel, he ran so fast and grabbed me from behind, both talking very calmly to

I ABSOLUTELY LOVED this FACILITY ❤ My mom was admitted and spent two weeks on the 2nd floor and staff was amazing ..They were very responsible in handling visitors as well as serving my mother in a timely matter.. The nurse and discharge nurse he wS amazing always answering questions and giving accurate information back to my family and I..The receptionist was professional and very helpful❤ It was great knowing my mother was in the best care after I left...I recommend this rehabilitation center to anyone they were my blessing at such a hard time in my life I couldn't be more satisfied ❤Thank

My mom was placed in this center after an episode in the hospital. I would not recommend this care center at all. There were many problems, but I will name the few that stood out. Trying to get ahold of someone was a joke, let alone having someone call you back. Uug. I never once received a return from my calls. The day time front receptionist should rethink her career path, what an unfriendly person. She must not like her job or something. As for the care, my mom was supposed to get her medication, but most days she was forgotten about for hours at a time. She would have to remind them then have

Abuse and/or lack of care from staff!
My grandma with Alzheimers was highly sedated during her stay but when my aunt (legally responsible) asked for a list of medications, they only had Tylenol listed. None of the nurses could tell her what medications were being used because only the doctor on staff knew and he was on vacation.
When my grandma was picked up (after threatening legal action because they wouldn't release her even after the legal amount they could require her to stay) she had soaked through her adult diaper and had smelled terrible. Definitely wasn't being changed nearly enough

Recovering from Covid-19. While at City Creek, I was surrounded by excellent staff and incredible care. I could not have recovered without such concern and personal care. The care team provided such over the top professional care and supportive personal support. What a great team. I am sooo thankful to them, and always will be.

Working here since November! I must say, this job has changed my attitude for the better on working since I started! I love working here, I love my coworkers, I love the residents. The management has accomplished so much with bettering our lives working here and living here at city creek!

The administration and their staffs were really nice to me. Their service was good, and they were very helpful to me in everything that I needed there. And maybe because of the coronavirus pandemic, that I didn't get much treatment that needed it, and I was okay with that. But I got my health back, I feel good and strong again. So thank you city creek rehab especially your staffs with names that I will never forget for great service.(and I miss some of you already 😂😂😂lol).....Sincerely T.
