Our licensed nurses will perform a comprehensive health assessment and work directly with your physician and our interdisciplinary team in developing a plan of care that ensures that all needs are addressed. They provide treatment and education to patients and their caregivers on the disease or injury process, the treatment plan, medications, diet and wound care.
Disabled Elderly Day Care facility seeks Health Program Manager to collaborate with other health professionals to evaluate disabled elderly clients' medical or physical condition to assess and manage client needs. Manage and refer disabled elderly clients to community resources to assist in recovery from mental or physical illness and to provide access to services.

The manger is a mess she’s very rude to the clients.. has major melt downs and tantrums..I have tired reaching out to her multiple times regarding my concerns.. she will not call you back.. some of the employees are rude and need to learn how to be more patient with the clients. Save your money and send your family to a better place.
