They sneak to the store to buy alcohol for the clients some cna's are leaving Patients by themselves soiled diapers the receptionist never answers the phone but when answer they hang up alot of fights and sleeping in the clients room cna's

Their physical therapy here is amazing! The staff was super kind and helpful through everything. The pandemic has made life a little bit difficult however the facility helped make everything a lot easier on my and my family.

SFHCR is great and the staff are friendly and patient. A relative went here after a fall and the staff took great care of her. The rooms were also very clean and well maintained. Definitely recommending this facility!

My partner has been hospitalized several times this year and last and discharged to an equal number of SNF's or "skilled nursing facilities" to recover. The worst of these was this one. After nearly a week in bed, not once did he receive a single hour of physical or occupational therapy or much attention from the nursing staff. If you find yourself in need of one of these facilities, avoid this one at any cost.

Horrible experience: I walked into this place and it smelled terrible it had a POTENT URINE SMELL. (I smelled it through my mask) The staff were extremely rude it was shocking.
The cherry on the top as I was waiting staff were walking out complaining about the poor people they were helping!

When I walked in the reception I was greeted by a cockroach. This facility is old dirty and shows no signs of maintenance. The food is horrible just horrible. I got my father moved out of there as quick as I could he hated it and fir the reasons stated. The sheets were torn the drapes were torn and the helps response time is horrible. If I could give this a zero I would. This place was chosen by the hospital as a transition. There isn’t one positive thing I can say about this facility.

The residents find new life here. It is unlike other facilities. We not only care about their physical health but also intergrate other aspects of their lives, such as spiritual, social, etc. making it a wholistic healthcare facility.
In addition, God specially promises that He will grow plants at the desert. Praise the Lord!

I am a middle aged person who needed services like San Francisco Healthcare and Rehab (SFHCR) provides. I was injured and was in the hospital for a short term but i needed long term care which SFHCR provided like physical and occupational therapy as well as 24 hour nursing care. Initially, I had a different idea and stigma of what long term care… facility look like. I was surprised that setting and environment look like any other hospital floor. I mean it was clean, bright, and full of busy staff and nurses roaming through the halls. My physical and occupational therapist were dedicated

Bad work environment for CNAs, Supervisor is very rude, The Owners have no compassion, it’s infested with roaches. They only care about the environment when State officials are there to inspect. Someone call the Health department. This place needs to be shut down!

Taking care of elderly people and making them happy - it is a tough mission.... The center made its mission to keep our dearest fragile and vulnerable mothers, fathers, spouses, etc. safe, well taken care off, entertained and comfortable. The staff provide physical therapy, painting therapy, watch medication intake, assist with hygiene procedures,… create caring ambiance and give emotional support to our loved ones. I had to make a very difficult decision to place my mom to the facility. I admit that when I discovered that the facility has well trained, knowledgeable staff and management
