Services
We are able to offer the same names and faces for our patients and partners due to maintaining our original team since our foundation in 2010. Upon request, one of our experienced registered nurses will evaluate any patient for home health eligibility free of charge. The evaluation is done at the patient's home or at any health care facility where the patient is located.
We have an ongoing research and development initiative that helps us tailor preventative programs in a way that reduces emergency care and unplanned hospitalization for our patients. We also help our patients become more independent in managing daily activities and treating symptoms by providing education to them, their families and caregivers.
Primary function is to develop, implement, maintain and evaluate an effective, data driven quality assessment and performance improvement program. Set priorities for performance improvement, consider prevalence and severity of identified problems, and give priority to improvement activities that affect clinical outcomes.

When a retired registered nurse uses Post-Health to recover from knee surgery and loves the quality of nurses & therapists it's a good day! The mother of my previous boss healed well and relatively quickly after seeing Post-Health for 8 weeks. Her 2 neighbors tried using Post-Health after seeing her results but couldn't due to the limited nature of their managed care plans.

As a former office employee, I would highly recommend Post-Health for any loved ones needs. The entire office works as a family to take care of your family. The behind the scenes atmosphere supports this well oiled machine to carry on all of its functions.
When the option presents itself to care for my parents, I will no go no where else but Post-Health.
