I am very fortunate to lead a team of highly committed and talented medical professionals. Charter is an entrepreneurial-minded group and since our founding in 2017 we have developed a number of successful services in various settings. One of my important responsibilities is to help the clinician - physician and nurse practitioner - develop a practice based on her or his interests and potential.
To get back on your feet following orthopedic surgery, a rigorous and targeted rehab regimen is key. We design an effective treatment plan in consultation with our therapists, doctors, your physician, and a board-certified physiatrist. Our goal is to help you regain your normal level of activity and return home as quickly as possible.
Behavioral health is an integral component of our care management model. Our diverse, interdisciplinary team are experts in patient-centered care for individuals with complex medical conditions and underlying mental illness. From screening and assessing depression to developing compensatory strategies to manage behaviors.

I hated it when I was there in 2017. Not enough nursing assistants, long waits for call bell to be answered, used agency nurses a lot who knew nothing about the facility or the patients, many times unable to get coffee, tea, or even ice outside of mealtimes. However, their physical therapy department was fantastic. The only reason I went back there a second time. A nd some of the nurses were fantastic. All in all

The residents really enjoy a variety of activities Louisa and her assistants arrange for them to partake in. I see a lot of smiles on their faces and hear a lot of much needed laughter.. Louisa is open to new ideas which is an added bonus. I’m so glad my mom is here because they take special interest in each individual.

My Mother has been very well taken care of while at the Brentwood. During these Covid times they have been eager to help families connect with Zoom calls, in person visits and assistance in making sure the residents receive what they need from inside and outside sources. The activities staff have been Awesome getting all the residents involved. Louisa, Zach and Florangel(?) have been so helpful in keeping my Mom busy and attended to during these lonely times. All the staff are very helpful and available to the whole family.

I have a family member within the rehabilitation unit of the facility. Before I get to praising the therapists and the management staff, it’s important to also praise the CNAs and Activities Staff. I am very grateful for the genuine compassion Louisa has given her. I am also grateful for nurse Mary, who was equally as compassionate.

Where do I begin. Brentwood was there for me and provided incredible and life changing medical services during my 4 month stay.
I suffered an accident which left me barely able to speak, eat, walk and take care of myself. Through physical therapy, occupational therapy, speech therapy and the kindness of each employee at Brentwood, I was able to regain my cognitive skills and get back on track.
Along with the nursing staff, the front desk staff, facilities staff (thank you Dustin) and food services were terrific. Also, I can’t praise the activities staff enough. Both Zack and Luisa kept me going

My mother has been at Brentwood for close to a month now after being discharged from the hospital due to Covid. My mother is 78 yrs young and yes a miracle to have made it to this wonderful facility. I can't say how wonderful the staff are not only with caring for my mother but keeping me informed about her status, as well as necessities that she made need from me. Leah and Karen have been wonderful in admissions and any time I call Linda is so pleasant on the phone to talk to. Even if I call and someone else answers the phone the they are so helpful and kind. All my needs have been met ! From the Nursing Staff to the orderly's who have been exceptionally helpful going the extra mile to make sure I was as comfortable as could be. The food was very good and I was able to even get extra coffee which I love to drink in the morning. The PT staff was Exellent. I have been exercising all my life and these folks have me instructions to speed up my recovery which has made me a very happy man.
Over all everyone has treated me with respect and dignity. I will be heading out of here sooner than than the Dr's expected.

I am so grateful to all the staff at Brentwood. My brother had surgery requiring after care at rehab. He went to Brentwood, 7E. He was only there about 4 days. This was due to the quick thinking of the Nursing Staff. They realized he had developed a rather serious post-operative complication that led them to the decision to send him to the hospital "immediately" rather than keep a "watchful eye" on it for a bit. The surgeon was grateful that decision was made as "it could have been disastrous" had they waited. He is now recovering. The staff at Brentwood is quite a team of special people. They
