Below you will find some basic essential forms needed to manage and administer your worker's compensation claim. If you are contending time-loss for a period of greater than 6 months, or in excess of $10,000, an Affidavit for Time Loss form may be required. If you receive a pension, a Declaration of Entitlement is required every year, usually around your pension anniversary date.
Workers' compensation pays for medical expenses resulting from an industrial injury or occupational disease. Workers' compensation may also provide monetary benefits to supplement income lost as the result of an injury or occupational disease. Employees do not have to prove negligence on behalf of the employer to receive benefits.

Another rollover from James Rolland who originally took this case. All Atlas law have to do is get checks to clients in a timely manner but they don't and they don't care. Now they are handing out checks in a Target parking lot . professional attorneys handing out L&I checks to their clients with disabilities in a parking lot! Did they notify us about this little arrangement? Nope. Got my information on a recorded message. If you can't pick up the checks they come much later in the mail. When they are late or don't come at all they blame the post office. The statements are innacurate but they blow

I came to Atlas when my employer was trying to close my claim. The folks representing my employer kept giving me the run around and sending me to doctors appointments where all the doctors just kept agreeing with each other. Atlas worked to keep my claim open and now I have a pension. I highly recommend!

Where to start. Ill try ti keep it short. I was a client of James Rolland who sold out to Elijah and became Atlas Law. So first they lied or Misinformed me about my paper work. They told me it was "just so Atlas Law can take over my case". I said cool and signed and went about my day. They did Not say they were taking more money from me. They took another 15% without saying a word to me. Youd think thats something they should bring up specifically and openly. I found later when i seen it on a payment check. The Worst part is that without talking to me at all, they took L&Is first offer. I broke

This attorney's office is a joke. They don't return phone calls. They haven't even been able to request a workers compensation file from the self insurance company. Somehow they just never actually sent the signed request over. Now because I had faith in attorney Elijah Forde that he could do his job correctly and in a timely manner I have to deal with this case on my own.

Elijah is great. And his staff are easy to talk to. I was reluctant to hire an attorney but it was a good call.
LNI spent the better part of three years after my injury delaying my treatment and my time loss payments. My case is still on going but now that I am working with Atlas Law I am getting my payments regularly and I finally got my surgery scheduled!
