We are a Home Health Care Agency located in the heart of Brooklyn Center. We have a variety of clients that include seniors, children, and people with disabilities. Angel's Health & Home Care Services serves all the 7 counties around the Twin Cities such as: Anoka, Hennepin, Ramsey, Dakota, Scott, Carver, and Washington.
We continue to grow our service areas outside the Twin Cities. This Agency believes that health care is a basic human right. It must be available, coordinated, and provided in a comprehensive way, combined with other human services when appropriate. To provide all levels of home care service in collaboration with physicians and other members of the health care team, allowing clients to remain in their own homes.

Services
Care Management Services are provided either by a Registered Nurse (RN) or a Licensed Social Worker (LSW). Companion/Caregiver service is companionship and assistance in activities such as meal and medicine preparation, transportation and completion. We provide home and community-based services (HCBS) to persons with disabilities and persons age 65 and older.
