Our private duty home care team provides a service to individuals who are in need of assistance with everyday activities. Those activities may include meal preparation, housekeeping, personal grooming, and hygiene. A private duty caregiver may also accompany you or your loved one to doctors' appointments and grocery shopping.
As a Deer Meadows Home Health and Support Services, LLC team member, you must be able to work independently and manage a busy schedule, as well as be efficient, organized, dependable, and punctual. Deer Meadows Home Health and Support Services, LLC is fully in compliance with the Americans Disabilities Act (ADA); we provide the best service without discrimination pertaining to the nature of the disability or illness-physical or mental.
Home care is care provided in the home when a patient is unable to can include both certified healthcare and personal care/private duty. As hospital stays decrease, increasing numbers of patients need skilled services when they return home.

In less than a week at this place, my mother development an serious infection after being admitted. This place almost killed my mother. Horrible place and Staff lies too your face about how they are giving your love one the best care. This place neglected my mother the short time she was there. If your love one is unable to move, or have limited use of their hands to call you. DO NOT SEND. When I finally saw my mother she was delirious and unresponsive and they were trying to get her to take medication for pain. The nurse did not notice until I bought it to her attention after insisting on seeing

Terrible. BEYOND terrible. The staff taking care of my mother treated her poorly. Disregarded her needs and went searching through her personal belongings in a box because they assume she stole one of their equipments when in actuality it was hers. They tried to with hold information that I needed on paperwork by giving me 2/4 pages. When I asked what happened to the other 2 pages they were trying to say that it wasn’t important information so I then said I still need them, regardless. They brought it to me and those 2 pages had all her personal information and how to use properly use her medicines

Mom came to my house for rehab after almost 2 weeks in the hospital. She lives in an independent apt in DM. DM home health is a separate entity from the facility. Their care has been excellent. I am very happy we chose them as the connection to her home has been a positive.I stress that they are a separate agency as I would not recommend DM rehab. The nurses and therapists, Holly, Toni, Gina, who have visited are professional and very respectful to my 85 year old mother. When she returns home next week, they will continue to follow her as needed.

I was at Deer Meadows from March til July social worker explained that Medicare only pays for 100 days, So they wanted me to pay $544.00 a day because they said I was 24hour care. Which I'm not , so I got discharged on July July 3rd. They wanted to send me home in an ambulance on a stretcher. My sister inlaw drove my car to me I jumped in the driver's seat and took her home. Now I want my medical records which Sharon Mikus refuses to release to me unless I pay almost $500.00 for. I told her I only need to pay if there going to a doctor or lawyer. I want them see t to me at my address. Now I'm filing

It's an honor to work with such a dedicated staff at Deer Meadows; Every part of their team from admissions to discharges and everything in between work together to ensure patient is well cared for -and safety measures taken to ensure nothing is missed upon discharge. Thus, ensuring each patient is sent home with the best of care plan possible in order for them to thrive at home. Very professional!

I am currently a patient at Deer Meadows. It is an excellent Hospital. The Doctors have been following my case very closely.
I am about to enter my 12th consecutive month of hospitals and nursing homes.
After being discharged from Nazareth Hospital, this is the only place in between a Hospitalization and home that has continued to follow through with determining the cause of my severe respiratory issues.
This has been a very scary year to me in regards to my breathing.
My lungs had collapsed during the suumertime and I have lost my ability to walk through severe muscular atrophy.
