Established in 2019, Independent Living Partners LLC is a chapter 245D licensed Home and Community based services provider. We are licensed to provide holistic list of services to support individuals at home and in the community. We are committed to our mission of providing professional services that result in greater independence for our clients and their families.

Services
Independent Living Services (ILS) help individuals increase their independence, meet community inclusion goals that are important to and important for the individual based on their assessed need and achieve their full potential. Individuals get training and supervision with communication skills, interpersonal skills, reduction/elimination of maladaptive behavior (as tolerated and directed by the individual) and self-care etc.
Housing Stabilization Services is Medicaid program aimed at helping people with disabilities and the elderly find affordable housing. A person's own home is a setting that he or she owns, rents or leases that is not operated, owned or leased by a provider of services or supports. The person has full control of his/her housing and choice of service provider.
Personal care assistance (PCA) services provide assistance and support for persons with disabilities, living independently in the community. This includes the elderly and others with special health care needs. PCA services are provided in the Minnesota Health Care Programs (MHCP) member's home or in the community when normal life activities take him or her outside the home.
Our leaders have over a decade of combined home and community services experience and have extensive expertise in CADI, BI and AC-EW waivers. With this expertise, we are able to network with broad based community resources to provide community connected and well-coordinated supports for individuals.
