We are located at 13410 Main Street in downtown Grabill, Indiana, one block south of Grabill Hardware. Our location is accessible to those living in northeast Indiana and northwest Ohio, and just 15 minutes from I69 & Dupont Road! Transportation is available through CTN for any CCHC services during the hours of 7:00am-6:00pm. Patients must register and schedule transportation in advance of the Clinic session. Services
ENT Specialty Clinics are offered monthly as needed in conjunction with our Acute Care Clinic. Preference will be given to patients referred through the CCHC Acute Care Clinic or by other area clinics. Patients are pre-scheduled for appointments in 15 minute increments. A limited number of walk-in patients may be seen as time allows.
Counseling sessions are offered monthly as needed in conjunction with our Acute Care Clinics. Counseling is available to patients of all ages throughout northeast Indiana and northwest Ohio. You are not alone. God cares for you and so do we! When you visit us for Mental Health Counseling Services, you will receive confidential and professional guidance from our licensed mental health counselors.
Christian Community Health Care began in 1996 as a medical outreach ministry to rural northeast Allen County, Indiana, and has now grown to serve patients throughout northeast Indiana and northwest Ohio!
