Community Home Health Care (CHHC) was established in 1995 to meet the growing demand for home based medical and support services. Now under new ownership, our team of vastly experienced, highly qualified health care professionals is committed to creating a new and higher standard of quality in the services we provide.
Professionals to take care of your medical needs and help teach self-care for your future independence under the supervision of a registered nurse. Registered Physical Therapists will teach and assist with exercises to help you regain strength, coordination, and mobility. Home care services for Developmentally Disabled Clients living in Group Home settings.
We are always seeking caring and competent individuals to become a part of our home care team. Since 1994, our organization has established a major presence in the Fort Wayne market-place as a leading provider of home health care services. The nurses and PA's from Community serve our house well. We couldn't ask for a more responsible and work-minded main PA and when she was off this last week, although we did have two different fill-ins, they picked up the specifics for each client quickly and were very proficient, friendly and proactive. Thank you for your help!
