After having our 85 year old Mom at OnPoint, I can not speak highly enough of this facility. The entire staff, from directors to nurses provide genuine care for their patients. I highly recommend OnPoint to anyone who needs care for their loved ones. Thank you everyone for taking such good care of our Mom!

Such a great place, I can't say enough about the Nursing care, they truly go the extra mile. My mom was there and it was a great experience. The care starts at the top all the way down. Any questions or concerns were address promptly and with a positive attitude. Nurse Faridah and Vincent and their aid Barbara were the best! If you want top care, this is the place to go.

I admitted my wife to Onpointe Dallas after a lengthy stay at 4th floor Hamon for a brain problem, and the staff and facility were great top to bottom. Special Kudos to Liz, Isabella, the PT and OT staff and other staff members for putting up with my wife and making her comfortable. If something happens to me in the future, I hope my wife sends me here for help too.

Gross negligence, incompetence, and indifference. My father has been here for over a month and has made NO progress due to all of the above traits. He got PT two days out of six this week. He hasn't had a bath/shower/wiping off in weeks. They leave him in the same clothes days at a time. They don't help him brush his teeth or shave. And he has lost muscle tone and strength as their idea of PT on the days he gets it is sitting him in a wheelchair for 2-5 hours at a time. Onpointe Translational Care is not where you would want to place your loved ones. The residents have to wait long periods of time for pain medication and they don't even get to see the doctor when they're admitted. The CNA'S ARE OVER WORKED and understaffed and the residentsd don't get the care that they need. They say that's an open door policy but it's not management will retaliated against you when you're doing what's right. They don't care if the staff is safe while having to transfer residents. I am posting this review to share my own story as a patient of OnPointe Transitional Care (OP). A few months ago I developed a sudden, severe infection of my knee. I live alone, still work in my profession, and, until the infection hit, was completely independent. The infection and the excruciating pain, and consequent surgery, left me unable to walk or to otherwise care for myself. My 76 year old mother required a SNF (skilled nursing facility) after a 10 day inpatient stay at UT Southwestern Hospital where she had surgery to have a tracheotomy placed due to cancer. As her adult son I had to find a facility that met several criteria which were: 1) A facility that would accept a tracheostomy patient (not all facilities accept these patients, 2) A facility that was COVID-19 free and where all patients were tested prior to admit, 3) A facility that was in-network with Humana Medicare and 4) A facility that would give her the necessary Respiratory, Physical Therapy, Occupational

My experience at On pointe was really good considering I was expecting worse. I was there for 3 to 4 months and during that time I was not the best patient but the nurses and p.c.t.s (patient care techs) really changed my mind. Two people that helped me alot were Demetria Slider and Nefreda a nurse. My visit there was tolerable and more enjoyable in the end. Thank you!!!!

I have been here for 7 days so far. I have been truly impressed with the staff and their of me. Nurses, techs, therapy and even custodial personnel are courteous, friendly and patient. Demetria and Liz have been outstanding in making sure everything is being taken care. I highly recommend Onpointe.

My experience at OnPointe was truly amazing it was like being home with family, from the doctors, nurses, receptionists at anytime you needed something they were there. Words cannot explain how happy this facility made me and my family knowing that we could go home and sleep instead of worrying was our mother ok ,but that's what families do take care of each other, thanks for being our home away from home

Very disappointed with this facility. Physical therapy almost nonexistent. Eighty percent of the time, staff does not answer the main phone line. Very difficult when you have a relative here with whom you cannot talk with, nor check on.

Check my mother in yesterday around 4. That was not very impressive. Now, less than 24 hours later I find out via a sister, that my 96 yr old mother fell. Nothing was said to us about it during a 3 way call to gather information that is NOT GIven to you in check in time. So far all I know that is she fell. We are getting the run around, put on hold and given vague answers. I want to know why when a 96 yr old women falls, and she has a broken collar bone, and hip a doctor is called right away. And now we can not even get a hold of anyone!

I want to send a special thank you to the wonderful staff here. They are true examples of angels sent always helpful and brightens your day. I have met several patients here as well who wanted to say thank you for getting them back on there feet so they can home. One patient in particular was here for therapy and had a Right Hip Fracture said the staff was very attentive to his needs and very friendly but he wasn't able to write because of his arthritis. He said from the Techs, nurses, therapy and everyone else truly made a difference in his life. He was able to walk right out of this place

Horrible Horrible facility.
Looks great care is subpar.
Can't get meds giving on time.
Food is no better than dog food.
Most of staff do not care.
Ring for help may show up an hour later.
Admin. Staff are totally clueless tell familys what they want hear but never follow thru.
State needs to investigate. How this place got a license to operate is beyond me.

My mother had a knee replacement and we had to choose a rehab facility for her recuperation. We chose Onpointe because it is focused on rehab, not long term care or assisted living.
The employees have all been very helpful and caring. They have been very attentive to my mom and she has made remarkable progress (according to her doctor). The therapists have been great in working with her and are patient and thorough.
All of the facilities we researched had bad reviews. As it turns out, most of them are understaffed, especially at night and weekends. This being said, all of her needs were met and

Bottom Line: RUN AWAY.

Where do I start? My family member was transitioned here from Arlington Memorial and steadily began to decline the moment he was admitted. I directly attribute this to the downright negligent care that he received (or did not receive).They do not follow generally recognized standard of care and best practices in multiple domains, including pressure sore prevention. They do not practice hand hygiene. Staff do not respond to call lights. And when they do, they do not implement care in a timely manner (or at all). They also failed, on multiple occasions, to properly respond

I'm still not sure how to feel about this place. Looks nice. Starbucks coffee shop, but it closes around 2 pm during the week and they're not even open on the weekends. Fairly new levels looking and modern decor. But... short staffed nursing. No supplies. No clock in my husbands room. Strange. We can a Care meeting after the first week and everyone seemed like they would be on top of everything. But, I haven't been contacted now in over a week to follow up. I've asked numerous times to speak back with the wound care doctor. She promised when I first met her over a week ago

I had my father here and they did a great job taking care of him. He had pneumonia, weak, and needed IV antibotics. The therapist was great!! Nursing staff was very attentive, caring, and pleasant. I would stay here myself and recommend OnPointe to others.

I had my father here and he was a complicated case. The staff, however challenged at times, was VERY caring and compassionate. They call themselves a Transitional care unit so we were moved to another facility that in comparison lacks the skills of this place. I would bring my father back at any moment.

This is the absolute WORST place! Unless they are able to get up on their own and care for themselves DO NOT SEND YOUR LOVED ONES HERE!! The staff is terribly uncaring. The nurses are only seen when passing meds and the aids are grossly inadequate (with the exception of a couple). Raphael Castillo the so called Clinical Manager is such a little weasel he won't even return phone calls to the the family after our mother died under their care. My mother went in healthy in need of one week of PT and she died 12 days later. She was sedated with Ativan and was like a zombie unable to get up

Unless you are a self-care patient and can fend for yourself, DO NOT choose this place for rehab. Incompetent and uncaring nursing staff with equally incompetent staff physician (Dr. Katherine Kenebrew). PT & OT were great, but nursing care and medical care was horrible. After a 10 day hospital stay at Presbyterian Hospital Dallas, my father was admitted to rehab care here, made significant progress during his first week, then developed an undiagnosed UTI (even though we now know there was indication of it in lab work).
