Evergreen Sport & Spine Physical Therapy (EVSSPT) opened its doors in 1996, in Evergreen, by Kimberly Kaiser, PT. 20 years later and more than 3 decades of practicing physical therapy Kim and her staff have landed in Conifer, Colorado. This clinic continues to serve a wonderful population of mountain loving clients from Conifer, Bailey, Evergreen, Morrison and surrounding foothill communities.
You do not need a prescription to receive physical therapy in Colorado. However, your insurance company may require you to obtain one in order to submit charges under your plan. You can have your physician's office fax your prescription to us, or you can bring it on your initial visit. Either way, contact our office to set up an appointment.
People in Colorado may access the services of Physical Therapists directly, without a referral or prescription from a doctor. However, your insurance company may impose certain policy restrictions that interfere in your ability to directly access a Physical Therapist. Evergreen Sport & Spine Physical Therapy participates with many insurance companies.

I have been a patient at Evergreen Sport and Spine for almost 20 years. I originally went due to a shoulder injury. They not only treated my shoulder, but also treated my whole body, helping me with my posture, flexibility, strength, and specific exercises to support my tennis game. A few years later, I was diagnosed with fibromyalgia and was getting injured very easily. I realized how much help I received from ESSPT for my injuries so when my therapy prescriptions were fulfilled, I continued to go back.
